Acute Generalized Exanthematous Pustulosis Induced by Paracetamol

Abstract: We report on a 28-year-old Bangladesh man with acute generalized exanthematous pustulosis, induced by paracetamol. The patient presented with an erythematous and pustular eruption after taking 1 tablet of paracetamol for a sore throat. After intravenous administration of propacetamol hydrochloride (which is a prodrug of paracetamol), the rash became worse, showing a toxic epidermal necrolysis-like appearance and the patient suffered from severe hemodynamic disturbances. After discontinuation of propacetamol hy… Show more

“…5 Dysfunction of multiple organ systems is rare, and only a few cases in the literature report hemodynamic disturbances. [6][7][8] We believe that the abnormalities observed on the initial chest scans for our patient were related to cardiogenic pulmonary edema with systemic inflammatory response syndrome-associated increased vascular permeability. The severity and prolonged duration of the inflammatory response in our patient probably led to cardiac dysfunction.…”

“…11,12 Definitive confirmation of a certain drug as the cause of AGEP theoretically can be obtained by rechallenging the patient with the suspected drug. In view of the possible recurrence of the full-fledged eruption, patch test can serve as a substitute mimicking AGEP both clinically and histologically at the patch test site only.…”

“…AGEP, TEN and hypersensitivity have all been described. [13][14][15] According to the Food and Drug Administration (FDA) adverse event reporting system database, 91 possible cases of TEN/SJS have been reported. 16 In addition, a pooled analysis of multiple case-control studies significantly associated paracetamol with TEN/SJS.…”
