Acute hepatic encephalopathy and multiorgan failure in sickle cell disease and COVID‐19

Abstract: To the Editor, Coronavirus disease 2019 due to severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) in sickle cell disease (SCD) is still being described, with vaso-occlusive crisis (VOC) and acute chest syndrome reported. 1 Reports suggest COVID-19 causes hyperinflammation, vasculitis, coagulopathy, 2 acute respiratory distress syndrome, thromboembolism, and multiorgan failure syndrome (MOFS). [3][4][5] We present a case of post-COVID-19 encephalopathy, hyperammonemia, hyperinflammation, and MOFS in … Show more

“…In the general population, children are less likely to experience severe COVID-19 outcomes as compared to adults [ 76 ], but only few studies [ 29 , 72 ] have examined children with SCD and SARS-CoV-2 infection in sufficiently large cohorts from which valid inference can be drawn, as the vast majority of studies were case reports with less than 10 patients [ 67 , 68 , 75 , [77] , [78] , [79] , [80] , [81] , [82] , [83] , [84] , [85] , [86] , [87] , [88] , [89] , [90] , [91] , [92] ].…”

“…35 Notwithstanding the variable outcomes reported, COVID-19 infection has been associated with severe disease in adult and pediatric patients with SCD. 31,32 VOE, 36 pulmonary embolism, 21 ACS, 19,22,37 splenic sequestration, 38 and acute hepatic encephalopathy with multiorgan system failure, 39 respiratory failure, intubation, and death have been all reported, 40,41 therefore, early and aggressive treatment is warranted.…”

“…The drug is used parentally for treatment of chronic iron overload occurs due to chronic blood transfusions used in treatment of B-thalassemia, sickle cell anemia and myelodysplastic syndrome [11]. The mechanisms of Deferasirox-induced hepatotoxicity may involve production of reactive intermediates by cytochrome P450, mitochondrial injury, and disruption of calcium homeostasis, stimulation of autoimmunity, canalicular injury and apoptosis [12].…”
