Hypersensitivity reactions to etoposide are reported infrequently and consist of hypotension, hypertension, flushing, diaphoresis, dyspnea, bronchospasm, and loss of consciousness. A 23-year-old woman experienced acute bronchospasm, tachycardia, hypoxia, and moderate hypertension minutes after an infusion of etoposide was begun. Symptoms resolved within an hour after administration of intravenous fluids, methylprednisolone, diphenhydramine, and oxygen. Subsequently, the patient was given etoposide phosphate without incident. To our knowledge, this is the first report of successful rechallenge with etoposide phosphate after an acute hypersensitivity reaction to etoposide.

Children suffering from cancer may experience short episodes of respiratory distress and/or chronic impairment in pulmonary function. Pulmonary dysfunction may be primarily disease-related, but it may also result secondarily from treatment. Emergencies with critical respiratory dysfunction in childhood cancer include mechanical obstruction of vital anatomical structures and hyperleukocytosis syndrome. This paper focuses on the most relevant causes of respiratory distress and lung injury in pediatric oncology patients and bone marrow transplant patients. Infectious causes, lung disease resulting from anti-neoplastic agents, and bone marrow transplant-related pulmonary dysfunction are emphasized. A review of the literature pertinent to this subject is given.

As reações de hipersensibilidade a antineoplásicos podem variar entre reações ligeiras a reações anafiláticas fatais e podem ser desencadeadas virtualmente por qualquer citostático. Na ausência de uma alternativa viável, a dessen-sibilização pode ser a única forma de prosseguir a terapêutica. Pretendeu-se com este artigo rever a literatura inde-xada publicada sobre este tema, de modo a caracterizar as reações de hipersensibilidade associadas aos fármacos citostáticos mais frequentemente utilizados em idade pediátrica e a sua abordagem.

Abstract: Despite this unexpectedly high incidence of hypersensitivity among Hodgkin's disease patients treated with etoposide, rechallenge with the drug was successful in 78% of cases.
