Acute mumps cerebellitis with abnormal findings in MRI diffusion-weighted images

Abstract: In a 5-year-old boy with acute mumps cerebellitis, abnormal findings on MRI diffusion-weighted images were transient with clinical improvement. High signal intensity in the cerebellum was obvious on diffusion-weighted images, suggesting the importance of diffusion-weighted images in the early stage of cerebellitis.

“…Areas of cytotoxic edema usually appear as regions of increased signal on DWI and have low ADC values relative to normal brain regions [16]. However, the lesions of the cerebellum with quick clinical improvements in our case, suggest an intramyelinic edema or inflammatory infiltration [10,15]. SPECT performed for our first patient showed a remarkable decline in cerebellar blood flow.…”

“…DWI abnormalities and SPECT were not described in these articles. Diffuse hyperintensity in the cerebellum on DWI is reported in acute cerebellitis patients with no abnormalities on conventional MRI [6,10]. Donmez et al reported a patient of acute cerebellitis that was diagnosed only on DWI by demonstrating lower ADC values [3].…”

“…La pleocitosis del LCR ocurre en la mitad de los casos de parotiditis. La meningitis cl铆nica se presenta en el 1-10% de las infecciones y la encefalitis en el 0,1% (5). La cefalea, el v贸mito y la rigidez nucal duran 7 a 10 d铆as.…”
