Acute Phosphate Nephropathy?An Emerging Issue

Abstract: Acute phosphate nephropathy is an accepted complication of the use of phosphate preparations in patients about to undergo colonoscopy. Age, renal failure, and the ongoing use of medications, such as angiotensin-converting enzyme inhibitors and/or angiotensin-receptor blockers, are now recognized as risk factors for the development of phosphate nephropathy. The presence of any of these risk factors necessitates careful attention to avoiding excessive dehydration in the process of bowel cleansing. In so doing, t… Show more

“…Un estudio encontró niveles de fósforo, entre los pacientes con función renal normal, que oscilaron entre 3,7 a 7,3 mg/dL, después de la administración de fosfato de sodio 7 . Otro estudio encontró niveles de fósforo superiores a 8 mg/dl en 28% de los pacientes tratados con fosfato de sodio 8 .…”

“…Additionally, a recent series of reports [7][8][9] described acute phosphate nephropathy followed by chronic renal insufficiency after taking NaP for bowel preparation. Nephrocalcinosis is the cause of renal injury and occurs when the concentration of phosphate increases, and calcium phosphate crystals are deposited in the renal tubules [9] .…”

“…Although this agent provides superior cleansing and is well-tolerated by most patients, there are concerns about its safety that are related to its osmotic action [5,6] . Moreover, recent reports [7][8][9] of renal injury associated with this agent have raised additional concerns. Given the mechanistic causes of the occurrence of adverse events, the use of reduced doses of NaP might decrease these potential risks.…”

“…7,8,12 Patient co-morbidity, particularly congestive cardiac failure (CCF), advanced liver disease and renal failure, has been suggested to be an additional independent risk factor. 13,14 However, there have been no reports of renal dysfunction or electrolyte imbalance with PEGbased oral bowel cleansing agents. 13 The recently published Consensus guidelines for the prescription and administration of oral bowel cleansing agents (jointly developed by the Royal College of Surgeons, British Society of Gastroenterology, British Society of Gastrointestinal and Abdominal Radiology, Renal Association and the Royal College of Radiologists) recommends an eight-step process for all patients when prescribing oral bowel cleansing agents.…”
