Adherence to Anticholinergic Therapy and Clean Intermittent Self-Catheterization in Patients With Multiple Sclerosis

Abstract: PurposeTo evaluate adherence to anticholinergic therapy (AT) and clean intermittent self-catheterization (CISC) in patients with multiple sclerosis (MS) and to identify factors associated with poor adherence.MethodsThis single-center study prospectively included 49 patients suffering from MS who had been prescribed AT and/or CISC. Adherence was evaluated using a self-report questionnaire. The Expanded Disability Status Scale (EDSS), Patient Global Impression of Improvement, Mini-Mental State Examination, Urina… Show more

“…Treatment of NDO are identical in HSP compared to SCI and MS, the first line being anticholinergics and if needed, intradetrusor injection of botulinum toxin A. Similar to what is observed in MS only 30% of patients performed CISC . We assume difficulties encountered in this population rely on the progressive evolution of the disease (and bladder symptoms) with a lack of adherence and acceptability to the technique …”

“…Similar to MS patients, an intensive education therapy program may be needed to allow them to understand the needs and benefits of CISC. Despite this, we often encounter difficulties to introduce CISC and obtain long‐term compliance to this therapeutic . This may explain why despite the importance of NDO‐DSD and chronic retention in our cohort, only 30% use CISC as micturition mode, often associated with spontaneous micturition.…”

“…Whilst numerous questionnaires on bladder symptoms are available, the USP was chosen as it is a validated questionnaire that incorporates different domains of LUT symptoms and better describes the frequently mixed symptoms experienced by patients with MS. Although not specifically validated in neurological patients, it has been recommended as a screening tool for LUT symptoms in MS [13] and used as an outcome measure in previous studies of patients with MS [14][15][16] and other neurological conditions [17,18]. A ratio of the USP LS/total scores was calculated to account for the degree of voiding LUT symptoms compared to a patient's overall profile of LUT symptoms.…”
