Adherence to Surgical Care Improvement Project Measures and the Association With Postoperative Infections

Abstract: Among hospitals in the Premier Inc Perspective Database reporting SCIP performance, adherence measured through a global all-or-none composite infection-prevention score was associated with a lower probability of developing a postoperative infection. However, adherence reported on individual SCIP measures, which is the only form in which performance is publicly reported, was not associated with a significantly lower probability of infection.

“…For example, implementation of Surgical Care Improvement Project (SCIP) measures in adult surgeries led to improved documented compliance rates with SCIP measures but did not necessarily yield improved outcomes in reduction of infections. 14,17 In this study, SCIP measures of preoperative antibiotic administration indeed correlated with improved outcomes in this pediatric population undergoing intrathecal baclofen pump surgeries. There was a significant difference between patients with documented overall bundled compliance with antibiotic prophylaxis and those without documented compliance.…”

“…Also before 2012, SSI surveillance relied on indirect identification via epidemiology followed by discussion with health care providers on just the NHSN-reportable cases. The Heart Center sought to track the total incidence of SSIs, not just ; a standardized preoperative inpatient and outpatient skin preparation protocol with either CHG or Sage Comfort Bath (Sage Products LLC), depending on the patient's age 4,11 ; education of outpatients' families and inpatient hospital staff on CHG or Sage Comfort Bath bathing requirements 11,12 ; standardized perioperative antibiotic (cefazolin) dosing, timing, and redosing (based on the patient's weight and gestational age) 4,[11][12][13] ; and multidisciplinary meetings after detection of every suspected SSI to discuss root cause and potential interventions. In early 2013, use of standardized silver-impregnated dressings, which are removed on postoperative day 7, was added for all postoperative surgical sites.…”
