Przegląd Dermatologiczny 2016/2 153 StreSzczenie Łuszczyca, jedna z najczęstszych przewlekłych chorób skóry, stanowi nie tylko istotny problem zdrowotny, lecz także społeczno-ekonomiczny. Wyniki leczenia tego schorzenia wciąż są niesatysfakcjonują-ce, na co wpływ ma poziom przestrzegania zaleceń terapeutycznych przez pacjentów (adherence i compliance). Jak wykazano w badaniach, im wyższy jest poziom przestrzegania zaleceń terapeutycznych, tym efekty leczenia są lepsze, a koszty terapii mniejsze. Niestety wyniki badań wskazują na niski poziom przestrzegania zaleceń przez pacjentów z łuszczycą, a zarówno chorzy, jak i lekarze wciąż nie są dostatecznie świadomi, jak dużą rolę w terapii odgrywają te elementy. Na podstawie przeprowadzonych badań wyróżnia się trzy główne grupy czynników będących przyczyną niestosowania się pacjentów do zaleceń terapeutycznych: czynniki dotyczące pacjenta i jego zachowań zdrowotnych, czynniki związane z terapią łuszczycy oraz czynniki związane z funkcjonowaniem systemu opieki zdrowotnej. AbStrActPsoriasis, being one of the most common, chronic skin diseases, is not only an important health problem but also a socio-economic issue. The outcome of psoriasis therapy is still unsatisfactory and is influenced by the level of patient compliance and adherence. Outcomes are better with a higher level of compliance, as demonstrated in the studies, and the cost of treatment is lower. Unfortunately, study results indicate a low level of adherence and compliance in patients with psoriasis, and patients and doctors are still not sufficiently aware of the role of these factors in therapy. Based on the studies, there are three main groups of factors which cause non-compliance of patients with treatment recommendations: factors relating to the patient and their health behaviors, factors associated with psoriasis therapy and factors related to the functioning of the health care system.Przestrzeganie zaleceń terapeutycznych przez chorych na łuszczycę -przegląd piśmiennictwa "Adherence" and "compliance" in the treatment of psoriasis -literature review

Psoriasis is a chronic inflammatory disorder effecting the skin and joints. Additionally, multiple comorbidities exist, including cardiovascular, metabolic, and psychiatric. The chronic nature of psoriasis is often frustrating for both patients and physicians alike. Many options for treatment exist, though successful disease management rests largely on patients through the application of topical corticosteroids, Vitamin D analogs, and calcineurin inhibitors, amongst others and the administration of systemic medications such as biologics and methotrexate. Phototherapy is another option that also requires active participation from the patient. Many barriers to effective self-management of psoriasis exist. Successful treatment requires the establishment of a strong doctor-patient relationship and patient empowerment in order to maximize adherence to a treatment regimen and improve outcomes. Improving patient adherence to treatment is necessary in effective self-management. Many tools exist to educate and empower patients, including online sources such as the National Psoriasis Foundation and online support group, Talk Psoriasis, amongst others. Effective self management is critical in decreasing the physical burden of psoriasis and mitigating its multiple physical, psychological, and social comorbidities, which include obesity, cardiovascular disease, alcohol dependence, depression, anxiety, and social anxiety.

Study Objectives:To evaluate factors associated with continuous positive airway pressure (CPAP) adherence in patients with obstructive sleep apnea (OSA) in the Apnea Positive Pressure Long-term Efficacy Study (APPLES) cohort. Methods: The data from a prospective 6-mo multicenter randomized controlled trial with 558 subjects randomized to active CPAP and 547 to sham CPAP were analyzed to assess adherence to CPAP during first 2 mo (early period) and during months 5-6 (late period). Results: Participants randomized to active CPAP had higher hours of nightly adherence compared to the sham CPAP group at both 2 (4.9 ± 2.0 h versus 4.07 ± 2.14 h, p < 0.001) and 6 mo (4.70 ± 2.08 h versus 3.41 ± 2.19 h, p < 0.001). Those assigned to sham CPAP were more likely to correctly identify their treatment group (70.0% versus 55.2%, p < 0.001). Irrespective of treatment group assignment, those who believed they were receiving active CPAP had higher hours of adherence than those who thought they were in the sham CPAP group at both 2 mo (4.91 ± 2.01 versus 4.17 ± 2.17, p < 0.001) and 6 mo (4.65 ± 2.10 versus 3.65 ± 2.22, p < 0.001). Among those randomized to active CPAP, older age was significantly related to CPAP use > 4 h per night. Presence of cardiovascular disorders was associated with higher hours of CPAP use, whereas presence of anxiety was associated with a trend toward lower hours of CPAP use. Presence of nasal congestion was associated with a decrease in mean daily CPAP use between the early and the late adherence period. The adherence during the week prior to a clinic visit was higher than the average adherence during the 2-mo period prior to the visit. Conclusions: Randomization to active therapy, belief that one is in the active treatment group, older age, and possibly presence of cardiovascular disorders are positively linked to CPAP adherence. Nasal congestion and anxiety are negatively associated with CPAP adherence. CPAP nightly usage increases as clinic visits approach.
