Adjuvant radiotherapy for the primary treatment of adrenocortical carcinoma: are we offering the best?

Abstract: Purpose:To evaluate the role of ARDT after surgical resection of ACC.Materials and Methods:Records of patients from our institutional ACC database were retrospectively assessed. A paired comparison analysis was used to evaluate the oncological outcomes between patients treated with surgery followed by ARDT or surgery only (control). The endpoints were LRFS, RFS, and OS. A systematic review of the literature and meta-analysis was also performed to evaluate local recurrence of ACC when ARDT was used. Results:Ten … Show more

“…The majority of patients was treated with conformal three-dimensional RT with only two patients reported to treated with conventional RT in the Germany study [9]. Two included studies did not describe the details on treatment technique, target volume, or treatment machine [10,12]. It was difficult to observe a relation between the LRFS rate with RT dose or RT…”

A total of 238 participants were selected for the meta-analysis, of which 111 and 127 patients underwent ART after surgical resection and only surgical resection, respectively.…”

“…Some indicated adjuvant radiotherapy may prolong the time to ACC local recurrence [35,36] , some even thought adjuvant therapy was associated with decreased recurrence [24] , others suggested it was of bene t in stage IV disease [32,37] . However, there was a consensus that it was no bene t for OS [24,35,38] . In our study, patients receiving adjuvant therapy didn't have signi cantly improved OS.…”
