Advances in noninvasive screening for renovascular disease

Abstract: • BACKGROUND Nearly 50 million Americans have hypertension, and renovascular hypertension accounts for perhaps 1% of them. • PURPOSE To review the current recommendations and the available screening tests for renovascular hypertension. • SUMMARY The presence of clinical clues increases the predictive value of screening tests for renovascular hypertension; these include abrupt onset of hypertension before age 30 or after age 55, severe hypertension, accelerated or malignant hypertension, hypertension refractory… Show more

“…For instance, medical treatment can be used to manage renal artery stenosis, either alone or in combination with revascularization. 43,44 Primary angioplasty can also be used as the primary management modality for cases with renal artery fibromuscular dysplasia. Angioplasty with stent placement can also be offered for patients with atherosclerotic diseases.…”

“…For functional assessment of RVH, captopril loading test (CP-T) [2] and captopril-enhanced 99m Tc-DTPA (diethylenetriamine-pentaacetic acid) renal scintigraphy (CER) [3] were employed. Our methods and interpretation of these examinations were reported elsewhere [4].…”

“…Thankfully, much progress has been made on diagnosis and management. We have now simple reliable tests for renovascular hypertension (RVH) [2][3][4] and diagnostic imaging modalities [5,6] that are less invasive than conventional arteriography, all of which could even make the diagnosis possible on an outpatient basis. Ever developing 25 interventional modalities are instrumental in rendering the revascularization procedure less invasive [7,8], while surgical revascularization (SR) has evolved over the years with improved technique [9].…”
