Advancing Patient-Centered Care in Tuberculosis Management: A Mixed-Methods Appraisal of Video Directly Observed Therapy

Abstract: BackgroundDirectly observed therapy (DOT) remains an integral component of treatment support and adherence monitoring in tuberculosis care. In-person DOT is resource intensive and often burdensome for patients. Video DOT (vDOT) has been proposed as an alternative to increase treatment flexibility and better meet patient-specific needs.MethodsWe conducted a pragmatic, prospective pilot implementation of vDOT at 3 TB clinics in Maryland. A mixed-methods approach was implemented to assess (1) effectiveness, (2) a… Show more

“…The innovative nature of the study intervention can be perceived as a strength. We are unaware of any prior published studies of video DOT via smartphone for buprenorphine alone without incentives to upload videos, although its use has been applied toward treatment of other diseases [ 40 , 62 64 ] and video DOT for buprenorphine with financial incentives is currently being investigated [ 65 ]. The randomized, controlled study design is a study strength, as is the enrollment at two different sites which enhances generalizability of results.…”

“…Video DOT utilizes the front facing camera of mobile devices to confirm proper medication ingestion and adherence. Video-based mobile technology for medication adherence has been successfully implemented in the treatment of active tuberculosis, cutting back on the need for face-to-face visits for monitoring [ 38 40 ]. Similar to treatment for tuberculosis, video DOT would obviate the need for directly observed, in-person administration of buprenorphine.…”

“…Both AiCure ® and emocha mobile health ® utilized AI technology for facial recognition to determine if a patient takes and swallows medication. 65 , 66 Furthermore, a few companies such as propeller health ® , Adherium ® and Cohero Health ® are manufacturing medication adherence devices to provide an objective measure of adherence. 67 69 These medication adherence devices have the potential to be used in assisting with improving the technique of inhaled medications.…”

“…The goal of the focus groups was to identify key areas of focus for the intervention, mobile app components, and to elicit feedback on wireframes ( Figure 2 ). Wireframes were based on areas developed by emocha Health, Inc for other adherence studies at a seventh-grade literacy level or lower [ 51 ]. Additional feedback was provided by an established sexual minority youth advisory board at the Philadelphia site.…”
