Introduction Chronic suppurative otitis media (CSOM) is a common problem in worldwide and untreated CSOM leads to fatal complications like facial nerve paralysis, lateral sinus thrombosis, labyrinthitis, meningitis and brain abscess in developing country like India. Objective To isolate causative bacteria and antibiotic sensitivity pattern for CSOM and to know the prevalence of extended spectrum beta lactamases (ESBL) and Metallobetalactamases (MBL) in CSOM patients. Methods A total of 500 ear swabs of clinical suspected CSOM patients were cultured on specific cultured medium and identified the bacteria with conventional methods. Then all the identified bacteria were subjected with specific antibiotics by the Kirby–Bauer's method to know the resistance pattern of antibiotics. ESBL and MBL strains were detected by double disc diffusion test. Results A total of 384 bacteria were isolated from 500 CSOM patients, among them 86 P. aeruginosa (22.40%), 112 Staphylococcus aureus (29.17%), 53 A. baumannii (13.80%), 32 E. aerogenes (18%), 26 C. freundii (6.77%), 24 K. oxytoca (6.25%), 23 P. vulgaris (5.99%), 18 K. pneumoniae (4.69%) and 10 P. mirabilis (2.60%) identified with conventional methods. From antibiotic disc diffusion methods 74.22% ESBL strains and 9.90% MBL strains were documented. Multidrug resistant strains of P. aeruginosa (86/384,22.40%) were more prevalent than those of S. aureus (112/384,29.17%) and other bacteria in ear discharges. Imipenem and vancomicin could control to gram negative bacteria and gram positive bacteria respectively. Conclusion Continuous and periodic evaluation of microbiological profile and antimicrobial sensitivity pattern of bacterial is essential for optimum management of CSOM patients.

In whole, 56 patients were included. Amidst identified microorganisms were fungus (4,7%) and bacteria (95,3%) to which Gram negative bacilli represented 72,1% (n=44), Gram positive cocci 6,4% (n=10), Gram positive bacilli 8,2% (n=5) and Gram negative cocci 3,3% (n=2). Among these bacterias, Pseudomonas aeruginosa and Proteus sp were predominant, with respectively 41% (n=25), 23% (n=14). However, three cases of S. aureus reported, six with negative coagulase Staphylococcus, one with Escherichia coli, one with Klebsiella sp, one with Haemophilus sp, two cases with Neisseria sp and four cases with Corynebacterium sp. Two types of cultures were noticed, one of them monomorphic (91,1%, n=51) and the other polymorphic (8,9%, n=5) to which 3 associations of P. aeruginosa-Proteus sp, 1 association of P. aeruginosa- coagulase negative Staphylococcus and 1 association of P. aeruginosa- E. coli. No resistance to ciprofloxacin was observed with Pseudomonas, Neisseria sp, Haemophilus, and enterobacteria except for E. coli. No resistance to rifampicin was observed with S. aureus. However, the sensitivity of S. aureus to ciprofloxacin decreased (one bacterium out of three). The use of rifampicin or fluoroquinolones should be based on the type of ear infections,

Background: Otitis Media is a prevailing and notorious infection in developing countries causing serious local damage and threating complication. Mainly in developing countries like Nepal, Otitis Media results because of illiteracy, poverty and poor hygiene. The aim of this study was to determine the profile of Otitis Media, its causative agents and their antibiotic susceptibility pattern. Methods:The study included 263 pus samples from 240 patients attending ENT department of Bharatpur hospital from May 2015 to January 2016. Samples were processed in microbiology department for bacteria using standard operating protocol. Antibiotic susceptibility testing was performed for all bacterial isolates by Kirby Bauer disc diffusion method and the results were interpreted according to clinical and laboratory standard institute (CLSI) guideline. Results:Out of 240 patients, 121 were female and 119 were male. Highest incidence of Otitis Media was observed in 1-10 year age group. Out of 263 samples taken from 240 patients, 216 showed bacterial growth. Gram negative bacteria predominated and the most common bacteria isolated were Staphylococcus aureus 36.11% followed by Pseudomonas aeruginosa 33.33% and Coagulase Negative Staphylococci 8.08%. All bacterial isolates were sensitive to gentamycin. Staphylococcus aureus was sensitive to Amikacin and gentamycin. All gram negative bacterial isolates were sensitive to Imipenem and gentamycin. 100% of Pseudomonas aeruginosa was sensitive to Imipenem. Conclusions:Staphylococcus aureus was the most predominant organism isolated from the pus swab followed by Pseudomonas aeruginosa and all the isolated organisms were sensitive to Gentamycin.
