Age, Atrial Fibrillation, and Structural Heart Disease Are the Main Determinants of Left Atrial Fibrosis Detected by Delayed‐Enhanced Magnetic Resonance Imaging in a General Cardiology Population

Abstract: Age, history of AF, and SHD are the most powerful predictors of atrial fibrosis, as detected by MRI, in a general cardiology population. Atrial fibrosis predominates in the posterior LA wall.

“…19,20 These simulations included interstitial fibrosis as microstructural discontinuities, 20 with distributions based on late-gadolinium intensity values from patients with persistent AF 21 used to infer probabilities for fibrosis inclusion in the model, resulting in heterogeneous anisotropic conduction. For 1 simulation, areas of fibrosis also included reduced conductivity and changes to the ionic properties.…”

“…138141 However, in a small study of women referred for cardiac MRI, more atrial fibrosis was detected in women than in men, using delayed-enhancement magnetic resonance imaging. 142 The plasma concentrations of the inflammatory marker C-reactive protein 143 and of fibroblast growth factor-23 144 have been found to be higher in women than in men in epidemiological studies. Both factors have been associated with increased risk of AF and might contribute to increased atrial fibrosis in women.…”

“…Left atrial remodelling, which can be assessed invasively by quantifying areas of low-voltage left atrial signals, or with MR imaging, 2124 is largely determined by concomitant cardiovascular conditions and possibly by duration of AF, while other factors were derived from clinical information. 25,26 It is worthwhile to consider that the effect of any AF ablation procedure is to induce further areas of delayed gadolinium enhancement. 23,27 …”
