Agminated Blue Nevi: Case Report and Review of the Literature

Abstract: Blue nevi may rarely appear in multiple form and grouped in a circumscribed area, a pattern of arrangement that is more properly designed under the term agminated blue nevi. In this paper a new case with light and ultrastructural studies is described, and the previously reported cases are reviewed. Histologically, there was a characteristic perifollicular arrangement of dermal melanocytes, most of which showed ultrastructurally an extracellular sheath. Agminated blue nevi seem to be benign lesions, but because… Show more

“…The few cases of agminated nevi described in the literature have been reported as plaque type, 3 eruptive, 4 and patch type, 5 corresponding histopathologically to common or cellular blue nevus. 1 Our case was clinically similar to one described by Hendricks 4 as an eruptive blue nevus.…”

“…Clinically, they present as a group of multiple blue nevi in a circumscribed skin area smaller than 10 cm in diameter. 1 The skin between the papules may not be discolored and may show a speckled or uniform blueblack or brown pigmentation. 1 We report a case of agminated blue nevus.…”

“…It sometimes appears in multiple form [28], grouped in a circumscribed area [29], in patch/plaque-type [26] as in our case, and may be misinterpreted as Spitz nevus or malignant melanoma [26,30]. At a microscopic level, it has a wedge shape, invariably involves the reticular dermis, and may penetrate the subcutis.…”

“…They can exhibit mild to focally prominent nuclear atypia, sometimes with mitotic figures [32]. Sparse to abundant melanophages are characteristic [32], corresponding histopathologically to an ordinary blue nevus or CBN [29]. Ferrara et al [2 ]described the case of an ‘atypical blue nevus' and Zattra et al [7] described a case associated with PTBN.…”

“…Progressive growth which is mostly related to the cellular type is rare, so is transformation to malignancy [1, 3]. Blue nevi typically occur as solitary lesions but can rarely be seen as multiple lesions, thereby grouped in a circumscribed body area [4, 5, 6]. Nongrouped disseminated, tardive manifestations of multiple blue nevi as in our case are extraordinarily rare.…”
