Agreement between transperineal ultrasound measurements and digital examinations of cervical dilatation during labor

Abstract: BackgroundTo compare 2D transperineal ultrasound assessment of cervical dilatation with vaginal examination and to investigate intra-observer variability of the ultrasound method.MethodsA prospective observational study was performed at Skane University Hospital, Lund, Sweden between October 2013 and June 2014. Women with one fetus in cephalic presentation at term had the cervical dilatation assessed with ultrasound and digital vaginal examinations during labor. Inter-method agreement between ultrasound and di… Show more

“…The only directly comparable measurement in our study was cervical dilation. We could measure cervical dilation in more than 90% of the women, which is a higher percentage than in previous studies . We believe that this high proportion was the result of all our cases having intact membranes and being in the first stage of labor.…”

“…Our findings demonstrate that there is a strong agreement in the decision to admit singleton pregnant women at term due to labor based on ultrasound measurements or digital vaginal examination. Measurements of cervical dilation are easily obtained by ultrasound and high concordance between observers has been demonstrated . Making decisions based on ultrasound and symptomatology instead of digital vaginal examination could result in a significant reduction in the number of digital vaginal examinations.…”

“…2 Because digital examination of cervical dilatation and transperineal ultrasound measurements are compatible with each other and transperineal ultrasound is less invasive and has a very good repeatability for women. 7,8 Authors used the following measurements on transperineal ultrasound to detect whether it is useful to predict vaginal delivery: HPD, head-pubis distance and AOP. The angle greater than 120º-126º was associated with vaginal delivery in many publications.…”

“…С помощью ТПУС производится оценка длины, структуры шейки матки, а так-же оценка риска развития истмико-церви-кальной недостаточности [7][8][9]. Ряд авторов указали на результаты мониторинга родовой деятельности, получение информации о сте-пени открытия шейки матки, продвижении, фиксации головки и ее размерах, как весьма важные для прогноза родов [10][11][12][13][14][15].…”
