All Arthroscopic Remnant-Preserving Technique to Reconstruct the Lateral Ankle Ligament Complex

Abstract: Arthroscopic lateral ankle ligament reconstruction has been recently advocated. But this technique has not been popularized because of the technical complexity and potential iatrogenic injury. Because the talocalcaneal and calcaneofibular ligaments are extra-articular structures, how to efficiently view and address them is a difficult task. Limited dissection outside the capsule to form a working space is required, but aggressive dissection is harmful for tissue healing although it is helpful for visualization… Show more

“…6 The ATTL can be observed arthroscopically. 7 There are already several reports of arthroscopic lateral ankle ligament reconstruction for the anterior talofibular ligament, [8][9][10][11] calcaneofibular ligament, [12][13][14][15][16][17] posterior talofibular ligament, 18 and lateral talocalcaneal ligament. 19 Here we describe the arthroscopic reconstruction of the ATTL using a free tendon graft (ARATTL).…”

“…In the past decade, ankle arthroscopy has been more and more frequently used in sports medicine. Thereafter, arthroscopic anatomic lateral ankle ligaments reconstruction procedures have been introduced [510]. The authors believed that arthroscopic anatomic reconstruction technique is reproducible and safe.…”

“…Several technical reports 5-9 and short-term clinical results [10][11][12][13] of arthroscopic lateral ankle ligament reconstruction have been published. Although the remnant-preserving technique has already been reported in 2 studies, 32,33 neither study repaired the remnant. On the contrary, the ATFL remnant and graft can share the load in our procedure because they are simultaneously fixed with the screw at the fibular tunnel.…”
