Allergy or Tolerance: Reduced Inflammatory Cytokine Response and Concomitant IL-10 Production of Lymphocytes and Monocytes in Symptom-Free Titanium Dental Implant Patients

Abstract: Hypersensitivity reactions to titanium (Ti) are very rare. Thus, we assessed the proinflammatory response and also potential tolerance favoring in vitro reactivity of human blood lymphocytes and monocytes (PBMC) to Ti in healthy individuals (14 without, 6 with complication-free dental Ti implants). The proliferation index (SI) in lymphocyte transformation test (LTT) and production of cytokines linked to innate immune response (IL-1β, IL-6, and TNFα) or immune regulation (IL-10) were assessed in response to TiO… Show more

“…Yttria-stabilized zirconia does appear to meet all important requirements as patients and dentists increasingly seem to prefer metal-free solutions (Sivaraman, Chopra, Narayan, & Balakrishnan, 2018). This trend is driven by growing concerns among patients about titanium sensitivity, allergies, or intolerance, even though there is scarcely any evidence that these factors might be clinically relevant (Fage, Muris, Jakobsen, & Thyssen, 2016;Hosoki, Nishigawa, Miyamoto, Ohe, & Matsuka, 2016;Thomas, Iglhaut, Wollenberg, Cadosch, & Summer, 2013).…”

“…Titanium particles have been reported to cause degenerative changes in peri-implant tissue where they are phagocytosed by macrophages and neutrophils activated as part of the biological response (Fage et al, 2016;Noronha Oliveira et al, 2018;Thomas et al, 2013). Although recent studies have demonstrated a potential of titanium particles to cause inflammatory reactions in peri-implant tissue, hard evidence for a causal relationship is yet to be provided (Fretwurst, Nelson, Tarnow, Wang, & Giannobile, 2018;Mombelli, Hashim, & Cionca, 2018).…”

“…In another study (Thomas, Iglhaut, Wollenberg, Cadosch, & Summer, 2013), 14 healthy individuals without titanium implants and six individuals with titanium implants without complication showed no enhanced reactivity in terms of lymphocyte transformation. In another study (Thomas, Iglhaut, Wollenberg, Cadosch, & Summer, 2013), 14 healthy individuals without titanium implants and six individuals with titanium implants without complication showed no enhanced reactivity in terms of lymphocyte transformation.…”

“…and TNFα release assay scores (Jacobi-Gresser, Huesker, & Schutt, 2013). In another study (Thomas, Iglhaut, Wollenberg, Cadosch, & Summer, 2013), 14 healthy individuals without titanium implants and six individuals with titanium implants without complication showed no enhanced reactivity in terms of lymphocyte transformation. In an interesting manner, individuals without implants showed higher cytokine response (IL-1β, IL-6, TNFα, IL-10) to titanium than individuals with symptom-free implants.…”

“…Однако существенная частота положительных реакций на красители у здоровых лиц указывает на возможные контакты с ними при употреблении пищи, лекарств, косметики и последующее развитие гиперчувствительности к ним. Так, TiO 2 способен индуцировать первичный иммунный ответ в культуре мононуклеаров с выделением цитокинов IL-1β, IL-6 и TNFα [34].…”
