Alterações Cardiopulmonares Ocasionadas pela COVID-19 e Atuação Fisioterapêutica: uma Revisão de Literatura

Abstract

O SARS-CoV-2 é um vírus que pertence à família Coronavírus. Possui altas taxas de transmissão e pode causar graves infecções respiratórias. Objetivando avaliar o que está sendo publicado sobre as alterações causadas pelo vírus SARS-CoV-2 no sistema cardiorrespiratório humano, assim como sobre as intervenções fisioterapêuticas no tratamento destas alterações foi realizada uma revisão de literatura com busca bibliográfica nas bases de dados: Scielo, Pubmed, PEDro e o site oficial da Associação Brasileira de Fisioterapia Cardiorrespiratória e Fisioterapia em Terapia Intensiva. Os descritores utilizados foram: “Infecções por Coronavírus”, “Betacoronavírus”, “Sistema Respiratório”, “Sistema Cardiovascular”, “Fisioterapia”, bem como estes termos na língua inglesa. Foram incluídas 39 publicações que contemplassem a fisiopatologia da COVID-19 e atuação fisioterapêutica, no âmbito hospitalar, para o tratamento dos pacientes acometidos pela patologia, sendo excluídos aqueles que estivessem incompletos ou que não abordassem o tema proposto. Foram consideradas publicações entre 2006 e maio de 2020. Os estudos apontam que a enzima conversora da angiotensina 2, presente em grande quantidade no sistema cardiorrespiratório, atua como receptora para o vírus explicando as alterações ocorridas no organismo. Além disso, aborda a importância da atuação do fisioterapeuta no tratamento dos pacientes contaminados para a reabilitação funcional e prevenção de possíveis complicações. Uma intervenção fisioterapêutica, desde que adequada e seguindo os protocolos de biossegurança, é indispensável para a reabilitação do paciente infectado pelo SARS-CoV-2, posto que suas técnicas conseguem recuperar a funcionalidade do paciente de forma global prevenindo, também, as limitações resultantes da COVID-19. It has high transmission levels and can cause several respiratory infections. Aiming to evaluate what is being published about the alterations caused by the virus SARS-CoV-2 in the human cardiopulmonary system, as well as about physiotherapeutic interventions in the treatment of these alterations, a literature review was carried out with bibliographic research in Scielo, Pubmed, PEDro databases and the official website of the Brazilian Association of Cardiorespiratory Physiotherapy and Physiotherapy in Intensive Care. The describers used were: “Coronavirus infections”, “Betacoronavirus”, “Respiratory system”, “Cardiovascular system”, “Physiotherapy”, as well as these terms in Portuguese. Thirty-nine publications were included, that contemplated the COVID-19 pathophysiology and physiotherapeutic performance in the hospital setting to the treatment of patients affected by the pathology, being excluded those that were incomplete or that did not address the proposed theme. Publications between 2006 and May 2020 were considered. The studies indicate that the angiotensin-converting enzyme 2, present in large quantity in the cardiorespiratory system, acts as a receptor for the virus, explaining the alterations that have occurred in the organism. In addition, it addresses the importance of the physiotherapist’s role in the treatment of infected patients for functional rehabilitation and prevention of possible complications. A physiotherapeutic intervention, as long as it is adequate and following the biosecurity protocols, is indispensable for the rehabilitation of the patient infected by the SARS-CoV-2, since its techniques are able to recover the patient’s’ functionality globally, also preventing the limitations resulting from COVID-19.
