Abstract: In this review we present a comparative analysis of alterations in hemostasis and blood coagulation during normal pregnancy with those in pregnant women with glycemic disorders (diabetes mellitus type 1 and 2, gestational diabetes).

Цель исследования. Изучение макроэлементного состава тканей матки, пуповины и плаценты, а также особенностей строения эритроцитов в системе «мать-плацента-плод» у женщин, беременность которых протекала на фоне сахарного диабета 1-го типа (СД-1), гестационного сахарного диабета (ГСД) и патологии щитовидной железы (ЩЖ) разного генеза. Материал и методы. В ходе исследования был изучен клинический материал 184 беременных, среди которых 154 женщины имели заболевания ЩЖ различного генеза, СД-1, ГСД и 30 беременных составили группу сравнения. Всем пациенткам проведено ультразвуковое исследование ЩЖ при помощи аппарата Aloka-5500, а также комплекс лабораторных анализов, включающих определение концентрации общего и свободного тироксина, трийодтиронина и антитиреоглобулиновых антител в крови обследуемых. Изучение тканевых макроэлементов матки, плаценты и пуповины проводилось при помощи растрового сканирующего электронного микроскопа Quanta 600 FEG, интегрированного с детектором рентгеновского излучения для микроанализа. Результаты. При изучении органогенных элементов в эритроцитах сосудов пуповины установлено, что значительные изменения в макроэлементном составе происходят за счет снижения содержания кислорода. Содержание натрия и калия в группах с патологией ЩЖ и СД-1 возрастало. Уровни магния, фосфора в артериях пуповины снижались в обследованных группах в следующей последовательности: эутиреоз, гипотиреоз, гипертиреоз, СД-1. Содержание натрия и кальция возрастало при дальнейшем уменьшении уровня магния и фосфора. Заключение. В результате исследования установлено, что при патологии ЩЖ и СД у беременной наблюдается маточно-плацентарная ишемия на фоне изменений в анатомо-функциональном состоянии эритроцитов и макроэлементного состава, следствием чего является развивающаяся деструкция в тканях системы «мать-плацента-плод», которая, в свою очередь, способствует дальнейшему прогрессированию тканевой и циркуляторной гипоксии.

The review provides information on the molecular genetic mechanisms for the development gestational diabetes mellitus (GDM). It gives data on the genetic identity of GDM and type 2 diabetes mellitus and considers a role of some adipokines and incretin hormones in the development of GDM. There is evidence for the role of magnesium and vitamin D deficiencies in the pathogenesis of gestational carbohydrate metabolic disturbances.
