Background and Objectives: Complementary and alternative medicines (CAMs) are generally considered non-scientific and poor effective therapies. Nevertheless, CAMs are extensively used in common clinical practice in Western countries. We decided to promote a Delphi consensus to intercept the opinion of Italian physicians on CAM use in clinical practice. Materials and Methods: We run a Delphi-based consensus, interviewing anonymously 97 physicians. Of these, only 78 participate to the questionnaire. Results: Consensus about agreement and disagreement have been reached in several topics, including indication, as well as safety issues concerning CAMs. Conclusions: The use of CAMs in clinical practice still lacks evidence. Experts agree about the possibility to safely use CAMs in combination with conventional medicines to treat non-critical medical conditions.

Complementary and alternative system of medicine for type 2 diabetes mellitus and its complications: A scoping review

Background: This review aims to map the evidence on the effectiveness of Complementary and Alternative Medicine (CAM) for Type 2 Diabetes Mellitus (T2DM) and its associated complications and identify research gaps in key outcomes and CAMs. Methods: We followed the Johanna Briggs Institute guidelines to undertake this scoping review. We carried out a comprehensive search from inception to March 2020 in electronic databases and registers. Forward and backward citations of included studies were also searched. The randomized controlled trials (RCTs) and non-randomized studies with intervention and control arm were mapped based on the effectiveness of various CAM (Yoga, Ayurveda, Homeopathy, Siddha, Naturopathy, Unani, and Sowa-rigpa) on outcomes among individuals with prediabetes or metabolic syndrome, T2DM with and without any comorbidities. Results: A total of 249 studies were included after screening 3798 citations. Most of the included studies were conducted in India and Iran. Most frequently studied CAM interventions include Yoga, Ayurveda, and herbal products as compared to other CAM therapies. Conclusion: Although we identified a lot of literature on the effectiveness of CAMs for T2DM, there is a lack of standardization of preparation of these medicines and the interventional modalities. To validate the potential benefits of CAM, robust research with high-quality RCTs and systematic reviews are needed to bridge the gap of evidence in CAM therapies. Future investments for research in CAM may focus on assessing the quality of existing trials and finding out an optimal dose, frequency, and duration of CAM, both as an alternative and complementary approach.

La homeopatía es una de las prácticas más utilizadas en la medicina alternativa, tanto en la atención de pacientes humanos como en veterinaria. Su origen se remonta a poco más de 200 años y su popularidad ha tenido altibajos desde entonces. Desde sus inicios y hasta la época moderna, los postulados de la homeopatía han sido motivo de debates en la medicina y en la ciencia. Conceptos aplicados en la homeopatía como el de similia similibus curentur, fuerza vital, dinamización, dosis mínimas o infinitesimales, y otras peculiaridades de la práctica, son poco reconocidos en la actualidad o incluso rechazados por completo. La debilidad del marco teórico de la homeopatía y la escasa evidencia seria que ha proporcionado dificulta su consideración como una herramienta de la medicina científica moderna. En la presente revisión de la literatura disponible, se describen de una forma crítica y rigurosa, los principios de la homeopatía, las evidencias experimentales de su funcionamiento, y su idoneidad en la medicina actual, para determinar si puede considerarse una práctica médica y veterinaria aceptable en el siglo XXI.
