Amniotic Fluid Embolism with Isolated Coagulopathy: A Report of Two Cases

Abstract: Case 1: The patient was a 29-year-old female with gravida 3, para 2, and abortus1 who had a spontaneous full term vaginal delivery. Her prenatal care was normal and intrapartum course was smooth with no use of pitocin. Spontaneous vaginal delivery with normal neonatal outcome occurred after membrane rupture. She developed profuse vaginal bleeding 1 hour and 10 minutes after delivery. The patient was transferred to our hospital one hour and 22 minutes after the bleeding episode. She presented with massive vagin… Show more

“…It has been reported that this event is frequent, 37 and several cases of AF embolism with isolated coagulopathy have been reported. 38 – 44 If AF is contaminated by meconium, the severity of coagulopathy and hemorrhage might significantly increase. In fact, we have observed that in patients with MSAF, the rates, both of moderate and severe hemorrhages were significantly higher than that of clear AF group.…”

“…This causes pulmonary vasoconstriction, pulmonary hypertension and subsequent failure of the right side of the heart [2]. Also, a coagulopathy may pre-exist or develop secondarily [3]. Resuscitation of patients with pulmonary hypertension is known to be difficult [4].…”

“…Previous concerns regarding risks of thrombosis have largely been disproven in randomized trials. [10,11] In a systematic review and meta-analysis, the use of TnxAc reduced all-cause mortality without increasing the risk of thrombotic complications. [12] In this case, does hypoxia caused by poor drainage of the drainage tube exacerbate thoracic hypoxia and cause endothelial cell damage leading to hypercoagulability?…”
