Achilles tendinopathy is a degenerative disease with high prevalence in runners. For the comprehension of achilles tendinopathy we have to consider intrinsic conditions such as age, sex, BMI, tendon temperature, systemic diseases, flexibility, previous injuries and anatomical variations, genetic predisposition and blood supply and extrinsic factors such as drugs and overuse. Both categories can be responsible for development of achilles' degenerative tendinopathy. Different hypothesis were studied, those concerning the pathogenesis of tendinopathy and those concerning the etiology of complaints in patients. This review of the literature demonstrates the heterogeneity of Achilles tendinopathy pathogenesis.

PurposeSurgical treatment of an acute Achilles tendon rupture can effectively reduce the risk of re-rupture, but it increases the probability of surgical complications. We postulated that a minimally invasive surgical treatment might reduce the number of complications related to open surgery and improve the functional results.MethodWe enrolled 47 patients with acute Achilles tendon ruptures in a prospective, randomised trial to compare clinical results and complications between a minimally invasive procedure with the Achillon® device and traditional open surgery with Krackow-type sutures. The average patient age was 46 years. The follow up time was 24 months.ResultsNo Achilles tendon re-rupture or nerve injury occurred in treated patients. There were two cases of wound infections in the open surgery group, and one superficial wound infection occurred in the minimally invasive group. The groups were not significantly different in the amount of pain, range of ankle movements, the single heel-rise test, calf circumference, or time to return to work and sports.ConclusionAfter a two year follow-up period, we found no significant differences in clinical outcomes between groups treated with traditional open surgery or minimally invasive surgery.

Abstract:The purpose of this study is to acquire mechanistic knowledge of the gastrocnemius muscleAchilles tendon complex behaviour during specific movements in humans through mathematical modelling. Analysis of this muscle-tendon complex was performed to see if already existing muscle-tendon models of other parts of the body could be applied to the leg muscles, especially the gastrocnemius muscleAchilles tendon complex, and whether they could adequately characterise its behaviour. Five healthy volunteers were asked to take part in experiments where dorsiflexion and plantar flexion of the foot were studied. A model of the Achilles tendon-gastrocnemius muscle was developed, incorporating assumptions regarding the mechanical properties of the muscle fibres and the tendinous tissue in series. Ultrasound images of the volunteers, direct measurements and additional mathematical calculations were used to parameterise the model. Ground reaction forces, forces on specific joints and moments and angles for the ankle were obtained from a Vicon 3D motion capture system. Model validation was performed from the experimental data captured for each volunteer and from reconstruction of the movements of specific trajectories of the joints, muscles and tendons involved in those movements.
