Medical care should be equally provided to the public regardless of their financial capability. In the real world, expenditures directly out from the patient sector decide the medical journey, even in a country with national health insurance. The aim of this study was to investigate whether there are differences in the diagnostic and treatment processes in hematologic malignancies based on patient characteristics, such as health insurance status.Through the review of 5614 "CBCs with differential count" results with abnormal cells from 358 patients from January 2010 to June 2017, 238 patients without past medical histories of hematologic malignancies were enrolled. Excluding reactive cases, 206 patients with hematologic malignancy were classified into 8 disease categories: acute leukemia, myelodysplastic syndrome, myeloproliferative neoplasm (MPN), myelodysplastic syndrome/MPN, lymphoid neoplasm, plasma cell neoplasm, r/o hematologic malignancy, and cancer.The patients' age, sex, disease categories and follow-up durations showed associations with the clinical course. The "refusal of treatment" group was the oldest and had a relatively higher percentage of females, whereas those who decided to transfer to a tertiary hospital were younger. The age, clinical course, and follow-up durations were different across health insurance statuses. The medical aid group was the oldest, and the group whose status changed from a medical insurance subscriber to a medical aid beneficiary during treatment was the youngest. The majority of patients who refused treatment or wished to be transferred to a tertiary hospital were medical insurance subscribers. The percentage of patients who were treated in this secondary municipal hospital was higher in the medical-aid beneficiaries group than in the medical insurance group. Follow-up durations were longest in the status change group and shortest in the medical insurance group.Almost all medical aid beneficiaries with hematologic malignancies opted to continue treatment at this secondary/municipal hospitals, indicating that this category of medical institutions provides adequate levels and qualified healthcare services to those patients. Background: Monoclonal B lymphocytosis (MBL) increases with age and individuals with high count MBL progress to chronic lymphocytic leukaemia requiring therapy at a rate of ~1%-5% per year. These cases usually have atypical lymphocytes at the microscope, abnormal representation in the scattergram, and positivity of flags. Using XN9000 (Sysmex), we noticed cases of MBL without this correlation. We studied customized gates for discovering MBL cases of our interest. Methods: We considered 212 peripheral blood samples with known phenotypes: 76.7% negative and 23.3% positive for B, T, or NK lymphocytes clones. We created gates studying the XN9000 FCS files in Diva software to identify new areas for better delimiting subpopulations of our interest and calculating sensitivity and specificity. Results: We found significant differences between negative and positive groups for Q-flag "Blasts/Abn Lympho?" (B/AL) and LY-X (p <0.05) with lymphocyte counts below 5×109/L. A new gate P1 normalized by P2 (P1n) differentiated between phenotypes much better than Q-flag B/AL with lymphocyte counts ≤ 5×109/L. Moreover, cases with MBL CD5 positive had higher medians (p <0.05). Conclusion: We propose a gate P1n as a new Q-flag for lymphocytes count ≤ 5×109/L, in order to hypothesize the presence of MBL CD5 positives.

A peripheral blood smear (PBS) report, which is prepared by observing the morphological characteristics of peripheral blood cells, is valuable for diagnosing various diseases. It is considered particularly important as a screening test for hematologic malignancies, such as leukemia, and a confirmation test for diseases that target red blood cells (RBCs), such as malaria and babesiosis. 1,2 While the PBS report furnishes substantial insights to clinicians, its interpretation demands a substantial investment of time and effort from hematopathologists.The International Society of Hematology (ICSH) published standard guidelines for the nomenclature and grading of peripheral blood cell morphological features for internationally consistent blood cell description in a PBS report. 3,4 Based on this, the Standardization Committee of the Korean Society of Diagnostic Hematology prepared detailed guidelines for standardizing PBS reports. 5 Despite these efforts for standardization, in the absence of an automated, standardized method that can easily apply the guidelines, the types of reports will vary owing to the different reporting styles of hematopathologists. This inevitably leads to communication difficulties between clinicians and laboratories and a decrease in the efficiency of patient care.The first step in generating a PBS report is to obtain complete blood count (CBC) results, which form the basis of the PBS report. Thereafter, the morphological examination of the PBS and other information, such as additional test results, diagnosis, age, and past history, are gathered to complete the PBS report. The description of the quantitative abnormalities in the CBC is a part of the PBS report. ICSH recommends using hematology analyzer-generated results rather than microscopic observation for the microcytes, macrocytes, anisocytosis, polychromasia, and quantitative WBC abnormalities in cases without abnormal cell populations. 5 We developed a standardized version of the PBS reporting format according to the ICSH recommendations and a system that allowed automatic descriptions of quantitative values of CBC data in this reporting format.
