An Electronic Decision Support Intervention Reduces Readmissions for Patients With Cirrhosis

Abstract: INTRODUCTION: Rifaximin use in combination with lactulose is associated with a decreased risk of overt hepatic encephalopathy (HE). METHODS:We prospectively evaluated the impact of an interruptive electronic medical record alert to indicate rifaximin for patients with cirrhosis and HE on lactulose. RESULTS:The intervention was associated increased rifaximin utilization, particularly for nongastroenterology and hospitalist services odds ratio 1.20 95% confidence interval (1.09-1.31). For patients with HE, the … Show more

“…Some examples of appropriate silencing of this alert were first HE episodes (as AASLD guidelines do not currently recommend rifaximin for the first episode of HE 4 ) or lactulose used for non‐HE indications. In this study, the overall cirrhosis readmission rate fell; however, readmissions deemed attributable to HE remained constant 40 …”

“…39 A similar study looked at an EMR best practice advisory that automatically alerted inpatients on lactulose with breakthrough episodes of HE to suggest the physician order rifaximin. 40 This could easily be ignored by physicians if they felt rifaximin was not indicated. Some examples of appropriate silencing of this alert were first HE episodes (as AASLD guidelines do not currently recommend rifaximin for the first episode of HE 4 ) or lactulose used for non-HE indications.…”

“…Of late, technology, among other measures, is being leveraged to prevent such unwanted and costly HE readmissions using various methodologies with some success. 72,73 One such health information (IT)-based technology intervention is the Patient Buddy App, which is an ongoing multicenter study, to find if IT can be used to decrease readmissions in cirrhosis patients. 73,74 However, such use and acceptance of technology are not limited to cirrhosis only but to other GI diseases also.…”
