Analysis of the delayed approach to the management of infected pancreatic necrosis

Abstract: This series supports the concept of delayed single-stage open pancreatic necrosectomy for IPN. Advances in critical care, antibiotics and interventional radiology have played complementary role in improving the outcomes.

“…A low incidence of repeated necrosectomies in our study may be attributed to an extensive use of step-up approach-utilizing image guided drainage of abdominal collections, thus postponing surgical debridement until complete demarcation of necrotic tissues. The rational for this particular strategy is endorsed by others [25] and is further supported by data from the randomized controlled PANTER trial [15].…”

“…19 Advances in critical care, antibiotics and interventional radiology have played complementary role in improving the outcomes. 20 Infants and toddlers with acute pancreatitis present with fewer classical symptoms and are managed differently from older children. 21 Patients of this study were treated conservatively in Intensive Care unit for massive support.…”

“…This is mainly due to the development of severe purulent necrotic processes in the parapancreatic tissue, which do not tend to delimit, sepsis, the development of intestinal and pancreatic fistula, the progression of multiple organ failure [2,5,7]. Currently, the role of minimally invasive surgical technologies, which are an important component of the complex treatment of infected pancreatic necrosis, is increasing [1,6,7,8,10]. They can reduce the effects of surgical stress of open interventions, stabilize the condition of patients and thereby delay the radical open surgical intervention and perform it at a more favorable time for the patient.…”
