Anatomic Plantar Plate Repair Using the Weil Metatarsal Osteotomy Approach

Abstract: The purpose of this study was to analyze the early results of a new technique for anatomic plantar plate repair and advancement using a Weil metatarsal osteotomy of the second metatarsal. The authors retrospectively reviewed medical records of 13 consecutive adult patients (15 feet) who underwent this procedure by a single surgeon. The authors recorded visual analog pain scale scores preoperatively and postoperatively and patient-reported clinical outcome measurement surveys during their final follow-up visit.… Show more

“…Surgical repair of plantar plate ligament disruption has evolved with increasing appreciation of the anatomy of the MTPJ (7,8). Primary repairs that involve direct visualization of the plantar plate injury through dorsal and plantar incisional approaches have been described (9-12), and procedures aimed at restoring MTPJ stability have been reported with the use of anchors (10), extensor tendon lengthening (13,14), flexor tendon transfers (14-17), crossover second toe repair (14,16), metatarsal osteotomy (11,18,19), and total joint implantation (20). However, considerable controversy still exists regarding the optimal operative intervention for this pathologic entity (12,17).…”

“…3,[8][9][10][11][12][13] Nevertheless, the dorsal approach often relies on complex instrumentation and it can only provide a limited view of the involved structures. 2,3,[8][9][10][11][12][13] Plantar plate tenodesis has been described to correct the claw toe deformity by stabilizing the attenuated plantar plate through suturing the plantar plate with the extensor tendon. [14][15][16] This can be performed under arthroscopic assistance, which can provide adequate visualization even without metatarsal osteotomy.…”

“…Concomitant procedures, for example, periarticular soft tissue release, extensor tendon lengthening, and Weil metatarsal osteotomy, can be performed together with repair of the plantar plate through the same dorsal incision. 3,[8][9][10][11][12][13] Nevertheless, the dorsal approach often relies on complex instrumentation and it can only provide a limited view of the involved structures. 2,3,[8][9][10][11][12][13] Plantar plate tenodesis has been described to correct the claw toe deformity by stabilizing the attenuated plantar plate through suturing the plantar plate with the extensor tendon.…”
