Anesthesia Considerations in Infective Endocarditis

Abstract: The management of infective endocarditis is complex and inherently requires multidisciplinary cooperation. About half of all patients diagnosed with infective endocarditis will meet the criteria to undergo cardiac surgery, which regularly takes place in urgent or emergency settings. The pathophysiology and clinical presentation of infective endocarditis make it a unique disorder within cardiac surgery that warrants a thorough understanding of specific characteristics in the perioperative period. This includes,… Show more

“…Coagulopathy in patients with IE is likely to be multifactorial [ 4 ], especially in the perioperative period [ 10 ]. Hence, ROTEM analysis cannot assess the whole picture of coagulopathy in IE, such as platelet dysfunction.…”

“…Based on the presented changes of the coagulation system, management of coagulation disturbances and bleeding complications represent particular challenges in patients with IE [ 10 ]. Intraoperative point-of-care coagulation testing (POCT), such as rotational thromboelastometry (ROTEM) may assist in decision-making concerning perioperative coagulation management [ 11 ].…”

“…American experts recommend early surgery, while European experts highlight the need to weigh the benefits of antibiotic completion before surgery against the risk of an uncontrolled source of infection. This lack of agreement in the guidelines highlights the paucity of evidence in the literature and has led anesthesiologists to advocate for multidisciplinary decisions, including surgical, anesthetic and ICU teams in a recent comprehensive review [5 ▪▪ ].…”

“…Despite these data, few well conducted studies or reviews are dedicated to critically ill IE patients. In addition, nearly half of IE patients undergo a surgical intervention [5 ▪▪ ], hence the perioperative management of these patients is of particular interest. This review will discuss the contemporary literature and will provide an overview of the current state of knowledge regarding the perioperative and intensive care of critically ill IE patients…”

“…Especially in the perioperative period, serious IE-related complications occur frequently and may have a significant impact on outcome [2]. However, the literature focusing on the perioperative care of patients undergoing surgery for IE is limited [3,4].…”
