Anesthetic considerations for robotic surgery

Abstract: Recently, demand for minimally invasive surgery has increased greatly. As a result, robot-assisted techniques have gained in popularity, because they overcome several of the shortcomings of conventional laparoscopic techniques. However, robotic surgery may require innovations with regard to patient positioning and the overall arrangement of operative equipment and personnel, which may go against the conservative nature of anesthesia care. Anesthesiologists should become familiar with these changes by learning … Show more

“…Hypothermia can occur due to prolonged operative time and should be treated aggressively using fluid warmers, warming blankets and mattresses (2). Temperature probe should be inserted not only to monitor hypothermia but also to ensure no over warming of the patient during the surgery.…”

“…The robot is rigidly fixed to its trocar insertion sites and over the patient's abdomen and chest or encroaching over the patients head, chest and abdomen. This invasion of the anaesthetic workplace leads to limited access to the patient's airway, monitoring devices and intravenous lines (2). After the robot has been positioned and engaged, the anaesthetists will have a very limited access to the patient.…”

“…As has been described [2]- [4], robotic-assisted surgery has many implications for the anesthesiologist. Patient hemodynamics and ventilation are affected by the pneumoperitoneum and degree of trendelenburg (or reverse trendelenburg) position needed for the use of a robotic surgical system.…”

“…Firstly, the wider gastrointestinal spectrum of procedures with many reconstructive steps has led to challenging learning curves with peculiar anesthetic implications. 6 Moreover, when applying criteria of Health Technology Assessment (HTA), RS has been shown to have higher costs related to the purchase and maintenance of technology and longer operating room time compared to the other approaches. 7 Lastly, the optimum level that has been achieved for some pure laparoscopic gastrointestinal techniques has caused much resistance to the development of a newer complex technology.…”
