Anesthetic Management Using Epidural Analgesia for Emergency Laparoscopic Cholecystectomy in a Patient with Lupus Anticoagulant Positivity and Prolonged Activated Partial Thromboplastin Time

Abstract: Lupus anticoagulant (LA), an antiphospholipid antibody, prolongs in vitro activated partial thromboplastin time (APTT) despite the presence of a hypercoagulable state in vivo. Irrespective of whether they receive antithrombotic therapy, meticulous anesthetic management is imperative in patients with LA positivity to prevent thrombotic complication. Additionally, emergency surgery in such patients can be challenging, as the time to devise perioperative strategies is limited. Here, we described the case of a pat… Show more

“…Isolated prolongation of APTT has various causes, including pre-analytical artifacts, interference of C-reactive protein, and inheritable bleeding disorders [ 4 6 ], and has medical significance as it can lead to the cancellation of surgery. On the other hand, even an emergency surgery utilizing epidural analgesia can be conducted in a patient presenting with severe APTT prolongation, provided careful evaluation and appropriate measures are taken [ 7 ]. Thus, it is not uncommon that anesthesiologists encounter preoperative isolated prolongation of APTT and the identification of the underlying etiology of the prolonged APTT is crucial.…”

“…Prolonged APTT indicates decreased or inactive coagulation factors, von Willebrand factor [7], prekallikrein [8], decreased activity of high molecular weight kininogen [9], and the presence of a lupus anticoagulant (LA) [10], [11]. Specific diseases and conditions include von Willebrand disease, hemophilia, disseminated intravascular syndrome, and Vitamin K deficiency [12], [13].…”
