Aneurysmal bone cysts: MR imaging at 1.5 T.

Abstract: Two patients with aneurysmal bone cysts of the pelvis were imaged using a 1.5-T magnetic resonance imaging device. Findings included multiple internal septations, cysts with fluid-fluid levels of varying intensity, and an intact rim of low-intensity signal completely surrounding the lesion. These findings allow a specific diagnosis of aneurysmal bone cyst to be made.

“…All were expansile and ranged in appearance from completely lytic to septated with a thin cortical shell. CT and MR imaging add important diagnostic information in the evaluation of aneurysmal bone cysts [3,4]. CT, because of its sensitivity to calcium, may show a thin rim not evident on radiographs, and helps to exclude calcified tumor matrix.…”

“…Both techniques may show fluid levels within the lesion because of layering of solid blood components [3,4]. Although fluid levels were often seen (seven of 1 0 patient studied with CT and/or MR), they are not specific to aneurysmal bone cyst, also…”

“…In computed tomography images of an ABC, the previous lesions can be observed, in addition to a visible aspect of cortical bone pressure trace and a thick bone crest (16). By contrast, MRI shows the clear boundary of the lobulated mass (22) and a characteristic liquid-liquid surface, particularly on T1-weighted images. In the present case, the signal intensity on the T1-weighted image was slightly higher compared with the muscle signal, while the T2-weighted image revealed an uneven cavity with a high signal, and a weak signal line between the sac intervals.…”

“…Small cysts called diverticula project from larger cysts. 6,7 In up to 85% of cases, it occurs by 20 years, with a mean age of 13 years. There is a slight female predominance.…”

“…11 Hemorrhages within the cysts of variable ages are present, causing fluid levels due to settlement of blood products. CT scans show an expansile, irregular lytic lesion of bone with cortical destruction with 7,12 Ultrasound can detect the fluid levels in a plane perpendicular to the sonic beam. It is an inexpensive, safe and reliable method, particularly useful in young children.…”
