Understanding the role of positive emotions in healthcare communication – A realist review

Aim To explore how the expression of positive emotions during the interaction between patients and providers can cultivate the patient‐provider relationship. Design We conducted a realist review guided by the Realist and Meta‐narrative Evidence Syntheses: Evolving Standards. Methods We systematically searched CINAHL, MEDLINE, PsychINFO and Scopus from inception to March 2019. Study selection and data extraction were performed blinded in pairs. From 3146 abstracts blinded in pairs, 15 papers were included and analysed. From each included paper, we extracted contexts, mechanisms and outcomes that were relevant to answer our research questions, creating a configuration between these elements (CMO configuration). Results Our findings suggest that in the contexts of person orientation and positive outlook, patient‐provider relationships improve by communication conveying and eliciting positive emotions. We found six underlying mechanisms for this that form either direct or indirect pathways between the context and the outcome.

Background Mothers of premature newborns in the neonatal intensive care unit (NICU) have complex needs and require a significant amount of support during the NICU admission. However, little is known about mothers' support needs in the NICU. This study aimed to explore health care staff and mothers' experiences of meeting the mothers support needs in the NICU. This study aimed to explore health care staff and mothers' experiences of meeting the mothers' support needs in the NICU. Methods A focused ethnographic approach was adopted. Observations and interviews with 21 mothers, 18 nurses, and five physicians were undertaken over a seven months period. Qualitative data analysis was conducted using the Roper and Shapira (2000) five-step framework. Result Two main themes of “insufficient provision of the mothers' support needs” (subthemes: inadequate accompany of the mothers in care, assigning monitoring and care to the mothers, inadequate sharing of medical the information) and “supporting the mothers in certain circumstances” (subthemes: reassuring the mothers, supporting the mothers with reduced functional capacity, providing information) were obtained. Conclusion The mothers experienced a gap between expected and actual support provided by health care staff. Although, the health care staff believed that mothers' support was a necessity, it was not their main concerns, and they considered workload as a barrier for the mothers support in the NICU.

Gaucher su bendraautoriais (2018) atliktame tyrime teigiama, kad su nėštumu susijusių seminarų lankymas moterims, kurioms nėštumo metu grėsė priešlaikinis gimdymas, padeda psichologiškai pasiruošti galimai gimdymo baigčiai [8], tačiau tyrime dalyvavusios moterys nėštumo metu informacijos poreikio apie priešlaikinį gimdymą neišsakė: "Žinojimas tikrai nepadės emociškai, kai tai įvyks. Tu gali nežinoti, bet tas jausmas, kai tu pagimdai likus pusei nėštumo, aš kad ir būčiau žinojus, tai manęs negelbėtų, tai ką man tenka patirti nuo pat pradžių."…”

Vaiko gimimas pripažįstamas kaip išskirtinis ir gyvenimą keičiantis įvykis, reikalaujantis jautrios paramos ir pagalbos, turintis įtakos trumpalaikei ir ilgalaikei šeimos gerovei. Šeima susikuria būsimo naujagimio paveikslą dar jam negimus. Neišnešioto naujagimio gimimas neatitinka tėvų lūkesčių, todėl šeimos patiria daug emocinių išgyvenimų ir turi taikytis prie naujų aplinkybių. Šeima baiminasi, kad vaiko būklė nepasunkėtų, abejoja savo gebėjimais rūpintis neišnešiotu naujagimiu bei įveikti kasdien vis naujas problemas. Siekiant atskleisti prieš laiką pagimdžiusių moterų patirtis, atliktas kokybinis tyrimas Lietuvos ligoninės Neonatologijos klinikoje. Tyrime dalyvavo penkios šioje ligoninėje pagimdžiusios moterys, kurių naujagimiai buvo gydomi Naujagimių ligų skyriuje ne mažiau nei dvi savaites. Atliekant tyrimą, taikytas pusiau struktūruoto interviu metodas. Tyrimo radiniai atskleidė, kad prieš laiką pagimdžiusių moterų nėštumas nesiskyrė nuo įprastinės nėštumo eigos, todėl staiga pasirodę priešlaikinio gimdymo požymiai tyrimo dalyvėms kėlė baimę, šoką, nežinomybę, mamos išgyveno planuotos nėštumo patirties praradimą, jautėsi nepasiruošusios gimdymui ir motinystei. Dėl staiga prasidėjusio priešlaikinio gimdymo, moterims teko keisti gimdymo planus, o naujagimio atskyrimas nuo mamos iškart po gimimo turėjo įtakos ankstyvai motinystės patirčiai − mamos prarado motinystės vaidmenį, gimęs neišnešiotas naujagimis neatitiko vaizduotėje susikurto naujagimio paveikslo, tačiau sveikatos priežiūros specialistų emocinė parama ir pagalba padėjo mamoms priimti naujagimį ir juo pasirūpinti.
