Background Anthropometric factors are reported to be risk factors for atrial fibrillation (AF), but it is unclear whether weight change in mid‐life is associated with AF. We aimed to study the possible associations of weight, height, and weight change with the risk of incident AF in men and women. Methods Our study cohort included 108 417 persons (51% women) who participated in a population‐based health examination in northern Sweden at 30, 40, 50, or 60 years of age. The health examination included weight and height measurement and collection of data regarding cardiovascular risk factors. Within this cohort, 40 275 participants underwent two health examinations with a 10‐year interval. We identified cases with a first‐ever diagnosis of AF through the Swedish National Patient Registry. Results During a total follow‐up of 1 469 820 person‐years, 5154 participants developed incident AF. The mean age at inclusion was 46.3 years, and mean age at AF diagnosis was 66.6 years. After adjustment for potential confounders, height, weight, body mass index (BMI), and body surface area (BSA) were positively associated with risk of incident AF in both men and women. Among participants who underwent two health examinations 10 years apart, 1142 persons developed AF. The mean weight change from baseline was a gain of 4.8%. Weight gain or weight loss was not significantly associated with risk of incident AF. Conclusions Height, weight, BMI, and BSA showed positive associations with risk of incident AF in both men and women. Midlife weight change was not significantly associated with AF risk.

Aims Taller stature has been associated with increased risk of atrial fibrillation (AF). AF and heart failure (HF) often co-occur but the association between height and risk of HF in older adults has not been well studied. We have examined the association between height and incident AF and incident HF in older adults. Methods Prospective study of 3346 men aged 60–79 years with no diagnosed HF, myocardial infarction or stroke at baseline (1998–2000) followed up for a mean period of 16 years, in whom there were 294 incident HF cases and 456 incident AF. Men were divided into 5 height groups: <168.2, 168.2–172.5, 172.6–176.9, 177.0–183.0 and >183.0 cms based on the 25th, 50th, 75th and 95th centiles distribution of height. Results CVD risk factors tended to decrease with increasing height but a positive association was seen between height and electrocardiographic QRS duration and incident AF. Both short stature (<168.2 cm) and tall stature (>183.0 cm) was associated with significantly increased risk of HF in age-adjusted analysis compared to those in the second height quartile [HR (95 %CI) = 1.62 (1.15, 2.26) and 2.04 (1.23, 3.39) respectively]. In short men the increased risk remained after adjustment for adverse CVD risk factors; in tall men the association was largely associated with AF and QRS duration. Conclusion Tall stature is associated with significantly increased risk of AF leading to increased risk of HF. Short stature was associated with increased HF risk which was not explained by known adverse CVD risk factors.

Introduction: care targeted at obese older people is a challenge for primary healthcare units, given the situation of doubled vulnerability imposed by old age and obesity. Objective:understanding the experiences of care received by obese older people in primary healthcare units. Method: phenomenological study conducted with 16 obese older people in a city in the state of Minas Gerais, Brazil. Phenomenological interviews were used to collect the participants' statements, whose organization and analysis were based on a theoretical study grounded on the social phenomenology of Alfred Schütz and literature on the theme. Results: the social phenomenology of Alfred Schütz allowed to identify difficulties related to geographical access, such as streets with heavy traffic, and to organizational access, such as the restriction of vacancies for medical appointments, long wait for scheduling laboratorial tests and for nursing service, in addition to the lack of healthcare staff, in the experience of the care received by obese older people in primary healthcare units. The care experience was found to be neglected, with general and specific instructions, instability in the anthropometric assessment, added to the perception of their obesity concealed by professionals in the appointments. These older people recalled group activities that were interrupted. Their expectations include ongoing care, being appreciated and listened by professionals with technical competence to intervene in their particularities; the availability of additional resources and professionals to favor their weight loss, and participation in educational groups.Conclusions: it is important to highlight the need to implement public policies and guidelines that may support the care of this population group at this level of care and for an increase in the qualification of human resources and the availability of structural resources, giving them the opportunity to receive comprehensive care, through a safe care routine, in the primary healthcare network.
