Anti-PR3 immune responses induce segmental and necrotizing glomerulonephritis

Abstract: SummaryWegener's granulomatosis (WG) is a life-threatening autoimmune vasculitis that affects lungs, kidneys and other organs. A hallmark of WG is the presence of classic anti-neutrophil cytoplasmic antibodies (c-ANCA) against selfproteinase 3 (PR3). Little is known about the role of these antibodies and PR3-specific immune responses in disease development. In this study, we demonstrate that PR3-specific autoimmune responses are pathogenic in nonobese diabetic (NOD) mice with an impaired regulatory arm of the … Show more

“…In fact, vasculitis could only be induced when splenocytes from these immunized NOD mice were transferred into NOD-SCID mice. 81 A more recent study using humanized NOD-SCID-IL-2Rg 2/2 mice injected with LPS and human IgG containing anti-PR3 antibodies showed the development of lung hemorrhage and mild kidney disease, characterized by mesangial hypercellularity and leukocyte influx. This supports the pathogenicity of PR3-ANCA.…”

“…138 Similar studies implicate an immune response to PR3 in pathogenesis. 139 Other models have utilized transfer of MPO + bone marrow, 140 adjuvants that enhance the immune response and increase cytokine levels, 141 and mice with subclinical GN immunized to human MPO in which the crescentic GN that follows is mediated by the immune response to endogenous MPO. 142 Studies of the Xaio model confirm neutrophil dependence and, despite the absence of antibody deposits, a requirement for alternative complement pathway activation involving C5a and C5a receptors.…”

“…First, homology between human and murine PR3 is less than that for MPO. Hence, animal models are more difficult to generate, and complex alterations in mice are required before achieving some vasculitic changes close to those seen in GPA (52)(53)(54). Second, only a fraction of ANCAs are pathogenic in an individual, i.e., only those ANCAs directed toward one or a few specific epitope(s) are pathogenic.…”
