Aortic dissection accompanied by preeclampsia in a postpartum young woman

Abstract: Aortic dissection is very rare in obstetrics, but it is a fatal disease. A 37-weeks primigravida woman with dyspnea and pitting edema presented to our emergency room. The patient was diagnosed with preeclampsia and underwent an emergency cesarean section under spinal anesthesia. The patient complained of severe dyspnea after the cesarean section, and the chest computed tomography scan was done. With the finding of aortic dissection, cardiopulmonary arrest occurred 5 hours after the cesarean section, and the pa… Show more

“…The systematic literature search yielded 465 potentially eligible studies. After exclusion, cross‐referencing, and reaching an agreement on three authors, 80 studies were included in this systematic literature review 5–84 (Figure 1). This resulted in a total study of 103 patients with aortic dissection during 6 months before delivery to 12 weeks postpartum.…”

“…Tachycardia, increased cardiac output observed during pregnancy, expanded intravascular volume and position dependent compression of the distal aorta and iliac arteries by the gravid uterus stresses the proximal aorta [ 4 ]. Preeclampsia is another risk factor often associated with aortic dissection [ 9 ]. In most cases of aortic dissection the histological findings show medial degeneration with mucoid degeneration and loss of elastic fibres.…”

“…Although the incidence is low overall, there is an increased risk of acute MI and aortic dissection in pregnant patients when compared to age-matched nonpregnant patients, particularly in pregnant and postpartum patients with a history of bicuspid aortic valve or connective tissue disorders. Hypertension, especially, is a risk factor for aortic dissection and patients like ours can present with preeclampsia postpartum [1,4,5]. PE accounts for 9.2% of all pregnancy-related deaths in the United States [6].…”
