Appendiceal intussusception due to a fecalith mimicking a submucosal tumor

Abstract: A 44−year−old man underwent a colono− scopic examination for a health evalua− tion. He had no subjective symptoms. A colonoscopy revealed a polypoid lesion 2.0 cm in diameter at the site of the appen− diceal orifice. This lesion had pink−col− ored, normal−appearing mucosa with a smooth surface and a 2−mm central dim− ple (l " Fig. 1). When pushed by a biopsy forceps, the lesion was found to have a hard consistency and could be partially re− duced into the appendiceal base. On the basis of the colonoscopic find… Show more

“…The receptor activities of compounds 2 and 3 were previously reported for the rat TRPV1/CHO system 10,13 and are reported here for human TRPV1 compared to the activities of RTX and I-RTX. Compound 2 proved to be a potent agonist for hTRPV1 with K i = 6.1 nM and EC 50 = 1.34 nM; it was thus within ca.…”

“…8 In order to find a simple surrogate of RTX, we initially analyzed the pharmacophores of RTX based on previous SAR investigations and proposed a pharmacophoric model in which the principal pharmacophores were the 4-hydroxy-3methoxyphenyl (A-region), C 20 -ester (B-region), orthophenyl (C 1 -region) and C 3 -keto (C 2region) groups. 9 On the basis of this model, we have extensively investigated simplified RTX surrogates embodying the principal pharmacophores of RTX to find potent agonists 9,10 as well as antagonists [11][12][13] These extensive efforts identified the template of N-(3-acyloxy-2-benzylpropyl)-N′-benzyl thiourea as an optimized surrogate of RTX. Compounds 2 and 3 represent the prototypic agonist and antagonist with high affinity as simplified RTX-based vanilloids in the rat TRPV1/CHO system.…”

“…1 The mechanism of appendiceal intussusception is thought to be abnormal peristalsis caused by local irritation, including irritation by a fecalith, foreign body, mucocele, endometriosis, or neoplasm. 2 The classification of appendiceal intussusception is based on which part of the appendix is the intussusceptum and where the intussusception is located. 3 Our case was classified as type III (Fig.…”

“…In this type, biopsy or endoscopic resection may provoke preforation. 2,4 Concerning the balance between surgical stress and the curative potential of resection in the case of malignancy, laparoscopic appendectomy with partial cecal resection is the best choice.…”
