Applicability and Prognostic Value of the New TNM Classification System in 135 Patients With Primary Cutaneous Anaplastic Large Cell Lymphoma

Abstract: To test the applicability and prognostic value of the new TNM classification system for primary cutaneous lymphomas other than mycosis fungoides and Sézary syndrome in patients with primary cutaneous anaplastic large cell lymphoma (C-ALCL) and to evaluate the prognostic significance of other clinical variables, in particular the site of presentation.

“…Given the higher incidence of cALCL in adults, most published series analyzing ALK expression have included predominately adult patients. 23 There have been only single case reports and small series of pediatric cALCL, and in these ALK staining was inconsistently performed. [24][25][26][27][28] We assume that our series is not population-based as cutaneous CD30 + LPD are diagnosed and treated either by dermatologists or pediatric oncologists.…”

“…2 Although LyP has a chronic course with recurrent self-healing papulonodular lesions, PCALCL manifests as rapidly growing and ulcerating skin tumors, 2 and .20% of patients with multifocal skin involvement die within 5 years. 3 Despite its usual indolent clinical behavior, LyP is associated with the development of second malignant lymphoma in nearly 20% of patients, including PCALCL and mycosis fungoides (MF). 4 Although not considered to be a CD30 1 LPD, MF may develop CD30 1 T cells, especially in the setting of largecell transformation (MF-LCT), which manifests a more aggressive clinical course.…”

“…Cutaneous relapses are common, but extracutaneous dissemination occurs in only 10% to 15% of patients and mainly involves regional lymph nodes. [2][3][4] LyP is characterized by a chronic course of recurrent, self-healing papulonecrotic or nodular skin lesions. Extracutaneous disease rarely develops.…”

“…3 The prognosis of both conditions is usually excellent with a 5-year disease-specific survival of approximately 90% for C-ALCL and almost 100% for LyP. [2][3][4] However, about 10% of patients with primary cutaneous CD30…”
