Approaches to Modifying the Behavior of Clinicians Who Are Noncompliant With Antimicrobial Stewardship Program Guidelines

Abstract: Antimicrobial stewardship programs (ASPs) are a key national initiative to promote appropriate use of antibiotics and to reduce the burden of resistance. The dilemma of managing the outlier physician is especially complex. We outline strategies to establish a successful ASP that reviews appropriate efforts to achieve the goal of modifying outlier physicians' behavior. One must try to differentiate deviation from ASP norms from all other issues of outliers. Essential elements include identifying and understandi… Show more

“…19 ASPs in community hospitals have unique challenges as compared with those at large academic medical centers. 20 The current study demonstrates that the use of prescriber peer comparison reports to reduce FQ use in a community hospital setting of various bed sizes is feasible and can be effective at altering prescribing habits.…”

“…One particular strength of the current study is its completion in a community hospital setting, which have historically been underrepresented in published literature on antimicrobial stewardship interventions . ASPs in community hospitals have unique challenges as compared with those at large academic medical centers . The current study demonstrates that the use of prescriber peer comparison reports to reduce FQ use in a community hospital setting of various bed sizes is feasible and can be effective at altering prescribing habits.…”

“…A major core ASP strategy supported by the IDSA and CDC is prospective audit and feedback (PAF). While effective, PAF is typically labor-intensive, difficult to implement in resource- and/or workforce-limited settings, and relies on provider acceptance of recommendations [ 31 , 35 ]. The current literature describes a wide variation in PAF designs that have attempted to overcome these disadvantages.…”
