Argon Plasma Coagulation in Flexible Gastrointestinal Endoscopy: Pilot Experiences

Abstract: These initial experiences indicate that APC seems to be effective in a number of indications, and relatively safe. Objective evaluation, a longer follow-up period, and comparative trials with other treatment modalities should follow.

“…EMR might be an excessive, relatively risky, and admissionrequiring procedure for the treatment of gastric adenoma with LGD. Since the development of an endoscopic applicator by Grund et al [1], APC has been used to treat a broad range of gastrointestinal problems, including bleeding ulcer [5], Dieulafoy's lesion [3], hemorrhagic telangiectasia [6], and tumor [7]. APC treatment coagulates the lesion, increases electrical impedance, and automatically prevents excessive tissue damage [1,2].…”

“…Argon plasma coagulation (APC) is a noncontact technique for tissue coagulation, which transfers high-frequency electric current through ionized argon gas to lesions [1][2][3]. Thermal effect of APC is limited to the mucosa and submucosa layers; however, its effect can be sometimes extended into the proper muscle layer of the gastric wall [4,5].…”

“…Colonic perforation especially right colon has been reported as high as 2.5% with thermal devices [15,95,96]. Distention of the GI tract with argon gas, submucosal emphysema, pneumomediastinum, pneumoperitoneum, and gas explosion has been reported as complications of ACP [95,97,98].…”
