Arthroscopic approach to the spring (calcaneonavicular) ligament

Abstract: The clinical relevance of this cadaver study is that it confirmed the feasibility of arthroscopic approach to the whole span of the spring ligament and alerted the potential risk of injury to the medial plantar nerve during arthroscopic assisted repair of the ligament.

“…21-23,27 Lui et al 15 highlighted the benefit to confirm spring ligament tears before surgical exploration to avoid unnecessary procedures and later presented a talonavicular endoscopic approach for spring ligament and Deltoid ligament repair. 16 Lui and Mak 17 then described an accessory portal placed plantar to the sheath. In our series, we use a dorsal accessory portal to allow us to move the posterior tibial tendon and get a better view of the spring ligament, avoiding the complication of sliding into the flexor digitorum longus tendon sheath or compromising the medial plantar nerve.…”

“…This can facilitate passage of the trocar-cannula and reduce the risk of medial plantar nerve injury ( Fig 5). 7 With the dorsolateral midtarsal portal as the viewing portal and the medial midtarsal portal as the working portal, the cartilage and subchondral bone of the medial part of the anterior talocalcaneal articulation are removed (Fig 6).…”
