Assessing risk for violence among psychiatric patients: The HCR-20 violence risk assessment scheme and the Psychopathy Checklist: Screening Version.

Abstract: This study evaluated the predictive validity of the HCR-20 (Historical, Clinical, and Risk Management) violence risk assessment scheme and the Psychopathy Checklist: Screening Version (PCL:SV). Files of 193 civilly committed patients were coded. Patients were followed up in the community for an average of 626 days. Receiver operating characteristic analyses with the HCR-20 yielded strong associations with violence (areas under curve [AUCs] = .76-.80). Persons scoring above the HCR-20 median were 6 to 13 times … Show more

“…This methodology breaks down individuals into groups based on the factors that best discriminate between recidivators and non-recidivators, so that a progressive two by two branching of risk factors and individuals into lower and higher risk groups occurs (Monahan et al, 2001). Nicholls, & Grant, 1999). Additionally, it is becoming more common for clinicians to utilize the HCR-20 in their risk assessments (Webster et al, 1997).…”

“…Although it is a structured professional judgement risk assessment instrument and clinicians are discouraged from summing individual item scores to produce a total score, for research purposes a total sore may be calculated. The predictive validity of the HCR-20 has been demonstrated in numerous studies (Douglas, Ogloff, Nicholls, & Grant, 1999;Gray, Fitzgerald, Taylor, MacCulloch, & Snowden, 2007; for review see Douglas, Shaffer, Blanchard, Guy, Reeves, & Weir, 2002.…”

“…141 Time for the HCR-20 can be reduced by omitting the short version of the PCL (PCL-SV), which does not add to the predictive value. 142 Stephen Hart maintains an HCR-20 research website that is regularly updated.…”
