The equestrian industry reports high rates of serious injuries, illness and fatalities when compared to other high-risk sports and work environments. To address these ongoing safety concerns, a greater understanding of the relationship between human risk perception, values and safety behaviours is required. This paper presents results from an international survey that explored relationships between a respondents’ willingness to take risk during daily activities along with, their perceptions of risk and behaviours during horse-related interactions. Respondents’ comments around risk management principles and safety-first inspirations were also analysed. We examined what humans think about hazardous situations or activities and how they managed risk with suitable controls. Analysis identified three important findings. First, safe behaviours around horses were associated with safety training (formal and/or informal). Second, unsafe behaviours around horses were associated with higher levels of equestrian experience as well as income from horse-related work. Finally, findings revealed a general acceptance of danger and imminent injury during horse interactions. This may explain why some respondents de-emphasised or ‘talked-down’ the importance of safety-first principles. In this paper we predominantly reported quantitative findings of respondents self-reported safety behaviours, general and horse-related risk perceptions despite injury or illness. We discussed the benefits of improved safety-first principles like training, risk assessments, rider-horse match with enriched safety communications to enhance risk-mitigation during human–horse interactions.

This retrospective cross-sectional epidemiological study deals with sport-specific injury patterns in show jumping. A total of 363 show jumpers of all levels (S) answered a retrospective questionnaire about injuries and overuse damages which occurred in the course of their careers. Demographic data and information on injuries in various body regions were collected. In addition to descriptive analysis, significance tests were performed. For better statistical comparability with other sports, exposure time was extrapolated with total career duration and weekly training hours, and injuries per 1000 jumping hours were calculated. The study included 251 (69%) women and 112 (31%) men, who were on average 26.9 ± 10.9 years old. The injury rate for the entire collective was 3.7 per 1000 h of exposure. The most frequently affected body region was the head (31%). Overuse complaints play a subordinate role and mainly affect the upper extremities (65%). The riders of the professional lower performance levels are less likely to injure themselves per 1000 h than riders of the higher performance levels. Riders who often or always wore a helmet suffered significantly fewer head injuries (p = 0.008) and had a significantly lower total injury duration than riders who did not wear a helmet (p = 0.006). Similarly, the study showed that riders who often or always wore a safety vest suffered significantly fewer spinal injuries (p = 0.017) and had significantly fewer injuries per 1000 riding hours (p = 0.031) than riders who did not wear a safety vest. Based on the present results, there should be an extension of the general helmet requirement and a requirement to wear safety vests in show jumping in general.

BackgroundCurrents medical advances are allowing patients with chronic arthritis to live to advanced ages. Although the risks of biological therapies in elderly patients have been previously evaluated, data are scarce since they are mainly derived from clinical trials, in which elderly populations are often underrepresented or event excluded (patients up to 75 years old). In addition, comparisons between such studies are difficult in the absence of a consensus in defining the groups’ age.ObjectivesTo evaluate the impact of age on the appearance of adverse events (AE) in patients with rheumatic diseases (rheumatoid arthritis -RA-, ankylosing spondylitis -AS- and psoriatic arthritis -PsA) at the start of biological treatment.MethodsMulticenter prospective study in a real-world setting. Information was obtained from BIOBADASER, a national safety registry of patients with rheumatic diseases treated with biologics or targeted synthetic disease modifying anti-rheumatic drugs. For this analysis, all patients included in this registry since 2000 and diagnosed with RA, AS or PsA were included, and classified into four categories according to age at initiation of biologic treatment: young (< 25 years-old), adults (25-64 years-old), elderly (65-75 years-old) and very elderly (> 75 years-old). Data collected included: 1) patient’s data 2) data on treatment and 3) data on AE. Proportions, means and standard deviations were used to describe the population. Poisson regression model was carried out to explore factors associated with the appearance of AEs. Crude and adjusted incidence rate ratios (IRRs) were calculated.ResultsA total of 2531 patients were included: 1154 RA (45.59%), 680 PsA (26.87%), and 697 AS (27.54%). Age groups: there were 64 young patients (2.52%), 2166 adults (85.57%), 243 elderly adults (9.6%), and 58 very elderly adults (2.29%). Comorbidities increased with age, while smoking rates decreased. Methotrexate use was similar in all age groups (53.99% in the total sample), but corticosteroid treatment increased with age (young 27.7%, adults 47%, elderly adults 64.36% and the very elderly 70.21%). 77.87% received anti-TNF treatment, and 22.13% other biological drugs. Poisson regression model showed an increased probability of suffering a first adverse event with increasing age regardless of the disease (IRR for AS: 1.04 (0.87-1.26); IRR for PsA: 1.08 (0.92-1.28)). Other factors associated with higher IRR were sex (being woman), type of biological treatment (TNF inhibitors), concomitant therapy (methotrexate), smoking, comorbidities (Charlson Index) and time of evolution of rheumatic disease. No differences were found between pathologies.TableResults of the Poisson regression model.
