Assessment of Interobserver Variability in Mitotic Figure Counting in Different Histological Grades of Oral Squamous Cell Carcinoma

Abstract: Mitotic counting is often used for classification, grading and prognosis of tumors. The count usually stands as a decision point for treatment as well. The easiest way of counting the number of mitoses is done by screening routine H&E stained slides. However, for proper mitotic counting, certain strict protocols should be taken into consideration. This study on 30 cases of different grades of oral squamous cell carcinoma was undertaken to determine the interobserver variations in two different groups: Grou… Show more

“…The detection of mitotic figures and determination of the mitotic rate per 10 high-power fields after staining with hematoxylin and eosin is an established marker of cellular proliferation in oncology 6 . The disadvantages of counting mitotic figures are the high inter- and intra-observer variation rates and limited reproducibility 10 , 11 . Another marker of cell proliferation proposed for the diagnosis of endometriosis is Ki-67 12 .…”

“…LM at 9400 magnification was used to identify mitoses. Features of mitosis and mimickers, as previously published, 6,[11][12][13] were applied to identify mitoses. To confirm mitosis and to differentiate between true mitosis and mimickers in challenging cases, phosphohistone H3 (PHH3) immunohistochemical (IHC) staining was used, as previously described.…”

“…The cells touching the inner vertical line belonged to the 5th column and cells touching the inner horizontal line belonged to the second row. The counting begun at the upper left hand corner of the section and was moved vertically downwards along each section till chondrocytes were found [19]. At the end of the vertical direction of the section, the section was moved in the horizontal direction.…”

“…This was important in reproducing the results. Ten high-power fields were counted and the average was taken [19]. The cells were counted by a consultant pathologist.…”
