“…However, an evaluation of the PSFHI programme in seven countries covered by the WHO EMRO indicated that none of the participating hospitals met the minimum compulsory standards required for the hospitals enrolled in the program (Siddiqi et al, ). Studies evaluating the status of patient safety friendly standards in Iranian hospitals show poor status of standards for attracting patients and community participation (Asefzade, Mehrabian, Nikpey, & Kianmehr, ;Mazhari & Adel, ) and has also shown that patient engagement is afforded little respect in the Iranian healthcare system (Atoof et al, ). Another qualitative study (Hooshmand, Tourani, Ravaghi, & Ebrahimipour, ) suggests encouraging patients' participation in the treatment process, advocating reorganization culture and concentrating on patient safety, patients' satisfaction and effective dealing with complaints to make the health system more patient‐oriented.…”

A review of studies showed that many patients receiving care in hospitals face complications or injuries arising from the provision of treatment services and a problem is added to an already existing problem ( 2 ).…”

“…Marderstein, Simmons, and Ochoa () reported that 38% had serious pulmonary complications among 57 patients with mechanical ventilation who had impaired nasogastric intubation. In another study in Iran, Asefzade et al () indicated that the status of patient safety in education and treatment centres of Rasht city was unsatisfactory. However, Janghorbani, Raisi, Dehghani, and Mousavi () in their study on the status of Shahid Beheshti Hospital operating room safety indicated that the level of patient safety standards was desirable.…”
