Assessment of Right Ventricular Function

Abstract: In contrast to studies suggesting that the right ventricle (RV) often functions primarily as a conduit for blood flow from the venous circulation to the lungs, recent data show that the chamber plays a major role in maintenance of global cardiovascular homeostasis under many conditions. Accordingly, clinicians involved with the perioperative care of surgical patients must be familiar with factors influencing RV performance. Comprised of two embryologically distinct regions, the inflow and outflow tracts, the c… Show more

“…Compared to other echocardiographic measurements of RV function, TAPSE offers several advantages in that it is highly reproducible, independent of geometric assumptions, and does not require RV endocardial tracing which is prone to error and technical difficulty. 16 In a prospective study of a heterogeneous population of PH, with approximately 75% in WHO Group 1, TAPSE < 1.8 cm was associated with a fourfold increased risk of death. 17 Follow-up TAPSE measurements, and not baseline, after initiation of PAH therapy predicted survival in a cohort of PAH patients.…”

“…Electrical activation of the RV-free wall spreads from the AV node via branches of the right bundle of the His-Purkinje system (Padala et al 2021 ) and is generally coincident with that of the LV although septal contraction may precede that of the RV-free wall. Within the RV, contraction is typically heterogenous with inflow tract contraction preceding that of the outflow tract by 30 to 60 ms, most likely reflecting at least in part regional differences in the conducting apparatus (Heerdt and Dickstein 1997 ).…”
