Objective: To determine the initial compensation, preoperative decompensation, and postoperative compensation of the lower incisors according to the skeletal anteroposterior discrepancy and vertical type in skeletal Class III patients. Materials and Methods: The samples consisted of 68 skeletal Class III patients treated with twojaw surgery and orthodontic treatment. Lateral cephalograms were taken before preoperative orthodontic treatment (T0) and before surgery (T1) and after debonding (T2). According to skeletal anteroposterior discrepancy/vertical type (ANB, criteria 5 24u; SN-GoMe, criteria 5 35u) at the T0 stage, the samples were allocated into group 1 (severe anteroposterior discrepancy/hypodivergent vertical type, N 5 17), group 2 (moderate anteroposterior discrepancy/hypodivergent vertical type, N 5 17), group 3 (severe anteroposterior discrepancy/hyperdivergent vertical type, N 5 17), or group 4 (moderate anteroposterior discrepancy/hyperdivergent vertical type, N 5 17). After measurement of variables, one-way analysis of variance with Duncan's multiple comparison test, crosstab analysis, and Pearson correlation analysis were performed. Results: At T0, groups 3 and 2 exhibited the most and least compensated lower incisors. In group 2, good preoperative decompensation and considerable postoperative compensation resulted in different values for T0, T1, and T2 (IMPA, T0 , T2 , T1; P , .001). However, group 3 did not show significant changes in IMPA between stages. Therefore, groups 2 and 3 showed different decompensation achievement ratios (P , .05). Group 3 exhibited the worst ratios of decompensation and stability (24% and 6%, respectively, P , .001). Anteroposterior discrepancy/vertical type (ANB: P , .01 at T0 and T1, P , .001 at T2; SN-GoMe: P , .01, all stages) were strongly correlated with relative percentage ratio of IMPA to norm value. Conclusions: Skeletal anteroposterior discrepancy/vertical type results in differences in the amount and pattern of initial compensation, preoperative decompensation, and postoperative compensation of lower incisors in Class III patients. The purpose of this study was to present a new approach for establishing the customized occlusal plane for an individual patient in systemized surgical treatment objective of class III cases. After the correction of incisor inclination to occlusal planes (U1-MxOP and L1-MnOP), the intermaxillary relationship can be established with normal overbite/overjet and maximum intercuspation. The anterior limits of the maxilla and the mandible can be positioned according to A to N perpendicular, Frankfort horizontal (FH) to AB plane angle, and upper incisor display. The vertical level of the upper first molar can then be determined in relation to the Y plane (an imaginary arch drawn using the distance between the lower incisor edge point and the rotation center point of the condylar head as a radius) and a triangle projected by the upper and lower limits of the FH to the occlusal plane (FH-OP triangle). Soft-tissue facial balance can be checked with the lip/chin projection in relation to the true vertical line at subnasale and the lower third facial height ratio. Appropriate superior impaction of the posterior part of the maxilla in relation to the Y plane and FH-OP triangle in case 1 produced significant improvement of the facial profile and occlusion. However, case 2 showed that the amount of superior impaction of the posterior part of the maxilla was not enough to produce a significant setback of the mandible and retraction of the lip and chin. The systemized surgical treatment objective flowchart that could set the customized occlusal plane for an individual patient might provide a useful guideline for obtaining optimal aesthetics and functional occlusion in class III cases.

Association between Orthopedic and Dental Findings: What Level of Evidence is Available?

Objective: The aim of this systematic review was to provide comprehensive access to and an analysis of the specialist literature published through December 2005 describing a correlation between orthopedic (leg-length inequality, pelvis obliquity, column diseases and head posture) and dental findings (occlusion, mandibular position, temporomandibular joints, masticatory muscles). Method: Four medical and dental internet sources (PubMed; Medpilot.de; databases of the Deutsche Ärzte-Verlag and Quintessenz-Verlag) were screened for relevant articles using carefully selected retrieval strategies and keywords. Bibliographies of relevant articles were examined for further pertinent publications. All relevant articles were tabulated according to their year of publication, the subject area discussed, and the levels of scientific evidence. Results: Our electronic inquiry yielded 359 relevant articles (electronic search: 195, search in bibliographies: 164), 355 of which could be analyzed. A correlation between dental findings and spinal column diseases was described in 266 articles, head posture in 216, pelvis obliquity in 53, and leg-length inequality in 35 papers. In 131 publications, conclusions were drawn from dental to orthopedic findings, whereas they were drawn from orthopedic to dental findings in 171 articles. The number of relevant articles rose significantly, particularly since the 1980's. Classification in levels of evidence reveals three publications (0.8%) with level II (randomized controlled trials), 63 (17.7%) with level III (experimental studies with no randomization, cohort studies, or casecontrol studies), 178 (50.1%) with level IV (non-experimental studies, such as cross-sectional trials, case series, case reports), and 111 (31.3%) with level V (narrative review or expert opinion without explicit critical appraisal). IntroductionSeveral publications in the German-language dental literature over the last few years have reported a functional relationship between orthopedic findings (spinal scoliosis, disturbances of the atlanto-occipital, atlanto-axial, and/or iliosacral joints, pelvis obliquity, leg-length inequality) and the masticatory system (occlusion, mandibular position, temporomandibular joints, masticatory muscles) [51,260,302,303,330]. The literature on this subject has originated in many countries (and languages) and is of variable quality, making assessment of the alleged associations difficult. We thus decided to carry out a systematic literature review. After compiling a list of relevant contributions published through December 2005, we analyzed their contents and quality.
