Association Between Bariatric Surgery and Macrovascular Disease Outcomes in Patients With Type 2 Diabetes and Severe Obesity

Abstract: IMPORTANCE Macrovascular disease is a leading cause of morbidity and mortality for patients with type 2 diabetes, and medical management, including lifestyle changes, may not be successful at lowering risk. OBJECTIVE To investigate the relationship between bariatric surgery and incident macrovascular (coronary artery disease and cerebrovascular diseases) events in patients with severe obesity and type 2 diabetes. DESIGN, SETTING, AND PARTICIPANTS In this retrospective, matched cohort study, patients with sever… Show more

“…Most previous studies analysing the impact of bariatric surgery in patients with T2DM focused on weight loss and glycaemic control, and had small sample sizes. A recent retrospective study from the USA found a lower risk of composite cardiovascular outcome in participants with diabetes undergoing bariatric surgery. After analysing the outcomes separately, these authors found a reduction in coronary artery disease, but a non‐significant difference in CVD, as in the present study.…”

“…A recent retrospective study from the USA found a lower risk of composite cardiovascular outcome in participants with diabetes undergoing bariatric surgery. After analysing the outcomes separately, these authors found a reduction in coronary artery disease, but a non‐significant difference in CVD, as in the present study. A meta‐analysis including five cohort studies found a 48 per cent reduction in macrovascular complications during follow‐up of 5–15 years, but no data were presented for individual components of macrovascular complications.…”

“…Improved glycemic control occurs days after RYGB, before substantial weight loss, supporting that weight loss-independent mechanisms drive metabolic improvements (11)(12)(13). Moreover, recent observational studies indicate that bariatric surgery is associated with fewer microvascular and macrovascular complications of diabetes and reduced mortality (14)(15)(16). Identifying molecular mechanisms responsible for improved systemic metabolism could allow development of novel nonsurgical approaches for T2D treatment.…”
