Association Between Marginal Jawbone Loss and Onset of Rheumatoid Arthritis and Relationship to Plasma Levels of RANKL

Abstract: Marginal jawbone loss preceded the clinical onset of RA symptoms, but this was observed only in nonsmokers. Moreover, marginal jawbone loss was significantly greater in RANKL-positive presymptomatic subjects compared with RANKL-negative presymptomatic subjects and was highest in presymptomatic subjects positive for both ACPA and RANKL.

“…Clinical studies have shown that, compared to patients with PD without arthritis, patients with rheumatoid arthritis have more severe PD with worse values in periodontal parameters. 14,15,17 It is important to note that, compared to the ART group, the greater amount of bone volume found in the ART/ASU group corroborates the previous findings showing that ASU promotes an improvement in periodontal repair, although not at the same level as that found in healthy animals in an earlier study. 10 It is important to note that the mainly statistical differences between the groups occurred at the early periods of evaluation, which may be related with the progressive periodontal repair observed after the ligature removal in this model.…”

“…The association of PD and ART has been investigated in cross-sectional studies, and ART has been considered a risk indicator for the presence of PD. 14,15 The difficulty of considering ART as a risk factor for periodontal disease is the fact that ART patients are usually under therapy with host response modulators, which modify PD progression. 25,26 However, experimental models that induce ART have been shown to increase bone loss in models of experimental periodontitis.…”

“…Moreover, severe periodontitis, defined as marginal jawbone loss, was significantly associated with the developed RA in a cohort of presymptomatic individuals (Fig. ), particularly in those who are ACPA positive and have increased concentrations of RANKL . Together, these data support the notion that periodontitis can be a source of citrullinated antigens and that periodontitis is a risk factor for the development of RA.…”

“…In this issue of Arthritis & Rheumatology , Kindstedt et al present data suggesting that periodontitis precedes the onset of RA . In this case–control study nested in a population‐based survey of cohorts in the Medical Biobank of Northern Sweden, radiographs were retrieved from dental offices before the occurrence of symptoms of RA in 45 presymptomatic subjects and 45 controls matched for age, sex, and smoking status who were selected at random.…”
