Association Between Postoperative Long-Term Heart Rate Variability and Postoperative Delirium in Elderly Patients Undergoing Orthopedic Surgery: A Prospective Cohort Study

Abstract: BackgroundPostoperative delirium (POD) is a common complication after orthopedic surgery in elderly patients. The elderly may experience drastic changes in autonomic nervous system (ANS) activity and circadian rhythm disorders after surgery. Therefore, we intend to explore the relationship between postoperative long-term heart rate (HR) variability (HRV), as a measure of ANS activity and circadian rhythm, and occurrence of POD in elderly patients.MethodsThe study population of this cohort was elderly patients … Show more

“…The incidence of POD was 26.7% in our study, which was similar to 27% in orthopedic surgery reported by a systematic review [20], and was also comparable with 20.4% in the aged patients undergoing similar type of surgery in a recent study [21]. It is widely accepted that the occurrence of POD was resulted from the complicated interactions of multiple predisposing factors.…”

“…The primary patient outcome is the occurrence of POD. The gold standard for diagnosing delirium is DSM5 (Diagnostic and Statistical Manual of Mental Disorders, 5th edition) [ 10 ]. However, the assessment approach was not documented in the raw data.…”

“…On the other hand, a lower proportion of REM sleep is thought to be related to the inflammatory response (Kane et al, 2022). In recent years, many studies (Sun, Zhang, et al, 2021) have also shown that the decrease in REM sleep proportion is related to delirium in ICU patients. However, both sleep and delirium in ICU patients can be affected by drugs, so it is difficult to clarify the relationship between sleep and delirium.…”
