Aim: In our study, we aimed to determine the clinicopathological factors affecting the pathological response after neoadjuvant chemotherapy in HER2 positive breast cancer. Materials and Methods:A total of 54 HER2 expression positive cases were included in this study. Neoadjuvant chemotherapy regimen containing trastuzumab was applied to all patients. Patients' age, gender, disease stage, tumor size and lymph node status, estrogen and progesterone receptor status, Ki-67 proliferation index, tumor grade, menopausal status and pathological complete response status after neoadjuvant therapy, neoadjuvant treatment regimen and the relationship between the tumor and histological subtype were examined.Results: Grade III tumor, hormone receptor negativity, high Ki-67 score, and the presence of T3 or T4 tumor were found to be better associated with pathological complete response (p=0.036, p=0.033, p=0.021, p=0.048, respectively). High tumor grade, hormone receptor negativity and high Ki-67 score were found as independent risk factors determining pathological complete response (p=0.043, p=0.047, p=0.035, respectively). Conclusion:In this series of 54 cases with HER2 positive breast cancer, the parameters determining pathological complete response after neoadjuvant treatment are high Ki-67 proliferation index, grade III tumor and hormone receptor negativity.

Background: The Ki-67, which is a proliferative index, became more important in making treatment decisions of breast cancers. But a few factors limited its use in clinical practices, especially the assessment of positive cells percentage of Ki-67 and the cut-off value of Ki-67. In this study, we examined the expression of Ki-67 by immunohistochemical method, and evaluated the value of Ki-67 index in patients with breast cancers systematically.Methods: The expression of Ki-67 was detected by immunohistochemical method, the relationship between over-survive (OS), disease free survive (DFS) of patients with BCs and Ki-67 index，and the value of Ki-67 in molecular classification of BC were analyzed with SPSS ver. 18.0 and GraphPad Prism software.Results: There were markedly difference between OS in patients with BC when the Ki-67 index arranged from 46% to 68% (χ2 = 6.87, P = 0.0088; χ2 = 7.69, P = 0.0056 respectively), and the same results were also found when the positive density was added to the Ki-67 index, but the positive density has limited value in assessment the value of Ki-67. There were markedly difference between DFS in BCs when the Ki-67 index arranged from 50% to 58% (χ2 = 7.31, P = 0.0068; χ2 = 7.88, P = 0.005). When using 14% as a cut-off point to classify the molecular type of BC, the Luminal A-type patients had statistical differences with patients with HER-2 over-expression, with triple negative type BC in term of OS respectively (χ2 = 4.8, P = 0.028; χ2 = 3.89, P = 0.049). There’s statistical difference between OS of patients with HER-2 over-expression BC when the Ki-67 index fell in the range of 49%- 60% (χ2 =4.97, P = 0.0259; χ2 =5.61, P = 0.0178, respectively). There were statistical differences between the Luminal A-type BC and HER-2 over-expression BC in term of OS (χ2 = 4.65, P = 0.031) according to suggestions of the CSCO consensus in 2019, there were statistical differences between two groups of Luminal B HER-2 (-) type BCs when the Ki-67 index was 52% (χ2=6.61, P = 0.0101).Conclusions: Ki-67 index can be used to assess the OS and DFS of patients with BC, and the positive density of Ki-67 has little value in assessment of prognosis in patients with BC. Different molecular classification methods may influence the assessment of prognosis and the results of molecular type in patients with BC.

The purpose of our study was to investigate the inhibitory effect of carboplatin injection on human neuroblastoma human neuroblastoma cell (SK-N-SH) cells and to clarify its action mechanism. In this study, human neuroblastoma SK-N-SH cells were divided into two groups. The treatment group was intervened by carboplatin injection (25 μM), while the control group was intervened by drug solvent. After treating separately for 24 and 72 h, the cells were collected, and western blot (WB) and real-time PCR were used to detect the expression of the proliferation marker protein (Ki67); cells grown on cover slips were prepared and immunocytochemistry (ICC) and hematoxylin–eosin (HE) staining were adopted to observe the protein expression of Ki67 and the morphological changes of the cells; clone formation assay was used to detect the clonality of each cell group. The cytotoxicity of carboplatin on SK-N-SH cell was checked by AlamarBlue viability test. Both WB and PCR results showed that after cells were injected with carboplatin for 24 and 72 h, the expression levels of both Ki67 gene and protein were decreased, and they had a significant difference from those of the control group. Carboplatin injection inhibited the expression of Ki67, and the inhibitory effect was particularly significant as the action time prolonged. ICC results showed that the protein expression of Ki67 in the treatment group was lower than that in the control group, and there was a significant difference in expression between them. As shown by HE results, the number of cell necrosis and apoptosis in the treatment group was significantly higher than that in the control group, while the results of clone formation assay showed that in the treatment group, after being injected with carboplatin, the proliferation ability of cells was inhibited, so the number of cells was significantly reduced compared with that of the control group. Carboplatin at the tested concentration displayed no cytotoxicity on SK-N-SH cell. The conclusions are that carboplatin injection can inhibit human neuroblastoma SK-N-SH cells, and the longer it acts on SK-N-SH cells, the more obvious the inhibitory effect would be.
