Associations of Osteoarthritis With Thrombectomy Utilization and Outcomes for Large Vessel Acute Ischemic Stroke

Abstract: BACKGROUND: Osteoarthritis and other musculoskeletal disorders are the leading causes of disability in the United States. While osteoarthritis is not a direct risk factor for stroke, osteoarthritis may impact patient selection for endovascular thrombectomy (EVT) due to prestroke disability. This study investigates associations of osteoarthritis with EVT utilization and outcomes. METHODS: This was a large-scale cross-sectional study of the 2016 to 2019 N… Show more

“…13 It is quite possible that the cause of pre-stroke disability influences functional prognosis after EVT. 34 In a subgroup excluding patients with mRS score improvement at 90 days, fewer patients with pre-stroke disability achieved FI or RoR than those without. Adding an axis of observation, such as whether the disability is fixed or recoverable, may contribute to a more sophisticated patient selection.…”

“…In a similar analysis of EVT for early stroke, cardiopulmonary disease and previous stroke were the most frequent causes of disability, followed by musculoskeletal disorder and cognitive impairment [ 13 ]. It is quite possible that the cause of pre-stroke disability influences functional prognosis after EVT [ 34 ]. In a subgroup excluding patients with mRS score improvement at 90 days, fewer patients with pre-stroke disability achieved FI or RoR than those without.…”

“…Our study predominantly utilized the ECI, comprising 31 comorbidities. However, careful interpretation is warranted, especially concerning conditions such as “paralysis” and “other neurological disorders,” which may be more aptly associated with stroke diagnoses rather than as separate comorbidities ( 16 ). Additionally, conditions such as “alcohol abuse” might be more fittingly categorized as stroke risk factors, while “fluid and electrolyte disorders” might be more aptly viewed as complications.…”

“…The Healthcare Cost and Utilization Project's National Inpatient Sample database was used to identify patients with LVO and the interventions performed from 2016 to 2020 using International Classification of Diseases, Tenth Revision (ICD‐10) codes using methodology similar to previous papers. 1 No ethics approval was required for this study as no sensitive data were used and all materials were collected from open, published sources.…”
